Responsible to ensure facility assigned to be in the best operational condition possible. To provide consistent supervision and training to assigned employees, to implement program services, individual supports and coordination of health services according to prescribed individual program plans and organizational requirements. Ensure the wants, needs, and desires of the people supported are carried out to the best of their abilities. A primary responsibility of all Calhoun County DSN Board staff is to support people with disabilities and special needs to achieve their personal goals, to help them develop and maintain relationships, skills, and roles which promote success in their lives.

Essential Duties

  • Demonstrating respect for people with disabilities.

A. Demonstrating respect for people with disabilities, their families, advocates, and volunteers.

B. Demonstrating an understanding of the organization’s mission and goals.

C. Demonstrating knowledge of and adherence to the organization’s policies and procedures.

D. Interacting, assisting, and encouraging to identify needs and desires with people served, their families, and volunteers.

  • Providing services and supports focused on people achieving goals.

A. Writes/reviews/evaluates individual’s program plan, referring to recommendations of both person served and previous performance and outcomes, makes adjustment and changes using discretion as to need in order to formulate individual program plan. Uses assessments, pre-staffing interviews/reports and monitoring as aids in developing plan.

B. Formulates individual objectives and evaluates individuals’ performance on assigned objectives examining outcome against stated criteria, using rating scales, tracking sheets, monthly progress notes, etc. in order to ensure personal growth and improvement in specific skills.

C. Substitutes for absent coworker in performance of assigned duties in order to ensure the day-to-day functioning of the facility.

D. Providing people with the opportunity to be a part of their community according to their personal preference and goals.

E. Ensure training and documenting objectives are complete as required by the plan.

F. Ensure assigned facility is safe and sanitary.

G. Assisting people to identify medical and dental needs and communicate these needs to ensure care is provided.

H. Promotes residents’ rights, assists individuals with making informed decisions, treats everyone with dignity and respect, reports suspected abuse or neglect, and supports independent expression, choice and decision-making.

C. Maintaining a cooperative and collaborative relationship with co-workers, other departments, subordinates, people served and their families.

D. Contributing both formally and informally in the assessment of strengths and weaknesses and the communication of needs and desires for people being served.

E. Working jointly with all service and program areas to support people’s needs.

F. Partners with Health Services Coordinator to ensure the medical needs of all individuals being served are met.

  • Maintain standards set forth by DHEC and CLC

A. Ensures all reported injuries of people served are documented as outlined by policy.

B. Communicates all emergency situations to families in a timely manner.

C. Ensures all drills (emergency, fire, and tornado) are reviewed monthly or when scheduled.

D. Completes timely follow-up on all assigned critical incidents and allegations of abuse.

E. Pursue all avenues to fill vacant beds within required timeframes.

F. Ensure assigned staff are completing their duties as assigned by their job description.

G. Conducts regularly scheduled staff meetings, using discretion as to subject matter, in order to coordinate service delivery to people supported.

H. Act as immediate contact person for Direct Support Staff and others.

I. Act as on-call administrator for assigned facility.

J. Completes all required paperwork thoroughly, timely and consistently. (ie monthly monitoring, pre-staffing, schedule facility inspections, call teams, and other paperwork as deemed appropriate by the program).

K. Ensure all vacant staff positions are filled as soon as possible.

L. Monitor the operational budget of facility assigned and make adjustments where needed.

M. Other duties as assigned.

  • Supervise Facility Coordinator

A. Hiring, disciplining coaching, and training of supervisory and nursing staff with the consultation of the VP ofOperations.

B. Provide day to day operational direction, guidance and support to supervisory and nursing staff.

C. Meet formally with supervisory and nursing staff on a monthly basis

D. Ensures all performance evaluations are completed as required on all employees.

E. Ensure all training requirements are met on an annual basis

  • Leadership of assigned facility:

A. Ensures all training is being monitored and documented as required.

B. Informs staff and family on information relating to the person served in a timely manner (hospitalization, injuries, abuse, critical incidents, changes in daily living skills, and special events).

C. Demonstrates willingness to try new tasks; generates new ideas for change; evaluates and recognizes priorities, selects effective team members, challenges others to learn, keeps current and integrates new information, communicates and models organization values, fosters high performance and recognizes the need for and provides adequate resources.

D. Demonstrates active listening techniques, gains support through effective relationships, treats others with dignity and respect, seeks feedback, sets clear standards for performance, evaluates job performance and provides effective feedback, establishes systems to measure effectiveness, efficiency and service and creates and maintains reporting mechanisms.

E. Responsible for accurately completing and submitting in a timely manner all documentation as required by individual.

F.Ensures staff coverage is provided during emergencies, staff shortages, weather emergencies, and other emergencies.

  • Minimum a four year Baccalaureate Degree in Human Services or related field.
  • Minimum of one (1) year directly related work experience.
  • Prefer at least two (2) years’ experience working with the developmental disabilities population.
  • Minimum of at least three (3) years supervisory experience preferred.
  • Must have strong coordination and problem solving skills and able to participate as a team member.
  • Flexibility and ability to work all shifts.
  • Must be able to lift 50lbs
  • Should possess the ability to complete written documentation, communicate effectively orally and have general computer skills to include but not limited to Microsoft Office.
