Position Summary: The Program Supervisor of the Medically Vulnerable Program (MVP) within the Homeless Services Division, will work with homeless individuals who have medical vulnerabilities that are a barrier to obtaining housing. The Program Supervisor will work collaboratively with the Recuperative Care Program (RCP) Case Manager, Medical Coordinator, and the Homeless Services Center Staff to ensure services are met and assist with client intakes and assessments of client needs. Supervisor will be responsible for monitoring compliance with goals through daily contact.

Responsible for distribution and ordering of needed basic supplies, as well as on-site Homeless Services Workers (HSW) & HSW RCP staff supervision. Works in partnership with the Homeless Services team to ensure that the dorm area is clean and safe for participant use, Ensure that participants needs are being met and that those who are enrolled in MVP Program are provided additional care. Coordinates with multiple service providers to enhance, augment, and coordinate service delivery to participants. Works with emergency services personnel, such as law enforcement and paramedics.

Supports CAPSLO’s mission by helping clients move towards increasing physical self-sufficiency leading to ultimate transfer into the general shelter population. Together with RCP Case Manager and Medical Coordinator, helps with maintaining files and completes all paperwork in a timely manner. Required Education and Experience: Requires a minimum of 2 years experience providing direct human or health services to medically related, economically disadvantaged, physically/mentally disabled, substance abuse/dually diagnosed individuals, the elderly, and/or at-risk youth. Education, volunteer experience, and/or other related experience may be accepted.

Preferred Education and Experience: Experience working with the homeless is preferred. License & Certifications: 1. Current CPR certification. 2.

Must have dependable insured transportation, and a valid California Driver License (mileage to be reimbursed), and acceptable driving record. A DMV printout and proof of insurance will be required. Essential Functions: 1. Responsible for hiring, providing orientation, training, and supervision to RCP Staff and Homeless Services Workers Overnight staff.

2. Responsible for staff work schedule, and arranges substitute staff when scheduled staff is absent, as directed by Interim Housing Manager. 3. Ensures timesheets are entered correctly into Ceridian, verifies hours, and checks for mathematical errors.

4. Oversees client data collection by RCP Homeless Services Workers (HSW) and HSW staff; maintains appropriate program records and other Health Care Services as required. Reviews and corrects staff paperwork to include charts, logs, medical progress plans, incident reports & rule notifications. 5.

Maintains a safe environment by continually monitoring the immediate site and premises. Enforces Homeless Services and site-specific policies and procedures. 6. Maintains a working understanding of the issues surrounding chronic homelessness and medical issues including; Post Traumatic Stress Disorder, Traumatic Brain Injury, alcohol and drug dependency, and other mental health issues.

7. Adheres to laws regarding confidentiality and reporting requirements; maintains knowledge of current HIPAA certification standards. 8. Maintains a professional relationship with clients and staff and conducts all business-related activity according to program policy and Employee Handbook.

9. Assists RCP Case Manager and Medical Coordinator with monitoring client behavior regarding program policies and goal achievement. Communicates with clients and shelter staff daily. Communicate with the Medical Coordinator and RCP Case Manager at a minimum, once weekly.

Ensures the RCP staff are in close communication with the Medical Coordinator and RCP Case Manager. 10. Assists with setup and paperwork when clients are moving in or out of medically Vulnerable Programs and coordinates bed assignments. Supervises RCP/MVP team in providing additional care for those enrolled in MVP, including but not limited to health education, medication support and education, enhanced linkages to health providers, and other needed supports.

11. Will coordinate coverage for open shifts if RCP staff are not available, or are out sick; will sometimes be required to cover open shifts. Is responsible for the maintenance and adherence to shift schedules for all RCP and overnight staff. 12.

Works with Homeless Services Program Supervisors. Lead in maintaining a clean and safe environment in dorms. Works with the Homeless Services team to ensure linens are changed regularly and that the linen room remains clean and organized. Responsibilities and Duties: 1.

Collects data as needed. 2. Must be dedicated to the goals and philosophy of the division’s child development programs and CAPSLO. 3.

Must use reasonable precautions in the performance of one’s duties and adhere to all applicable safety rules and practices; and act in such a manner as to assure at all times maximum safety to one’s self, fellow employees, clients and children. 4. Performs other duties as needed. Knowledge, Skills and Abilities: Knowledge 1.

Knowledge of local human services delivery system. 2. Must be computer literate with basic knowledge of Microsoft Word, Excel and data tracking programs. Will be expected to learn Homeless Management Information System (HMIS) within 30 days of hire.

Skills 1. Must be proficient in charting and other documentation required. Abilities 1. Ability to meet and deal pleasantly and effectively with homeless individuals.

2. Must be able to relate well to all people of the community regardless of color, national origin, religion, sex, pregnancy, age, marital status, veteran status, sexual orientation, disability or socio-economic level. 3. Must understand, uphold, and continue working toward accomplishing the mission, strategic goals and performance measures of the agency.

Supervisory Responsibility: This position provides direct supervision to Homeless Services Workers (HSW) & RCP Homeless Services Workers, approximately 6-8 staff. Work Environment: 1. Requires working indoors in temperature-controlled environment with some exposure to 2. The work environment may include some exposure to aerosol transmissible diseases (ATD).

Position Type/Expected Hours of Work: 1. This is a regular, fulltime 40 hours per week position with additional hours as needed. 2. Must be available to work occasional evenings or weekends.

Vacation/Sick Leave CAPSLO awards 80-160 hours of vacation annually with a maximum accrual of 320 hours, depending on years of service.

Paid Sick time is accrued at 8 hours per month (based on weekly hours) with a maximum accrual of 280 hours.
