PeaceHealth is seeking a Program Assistant Diabetes Care and Education for a Full Time, 1.00 FTE, Day position. The general salary range for this job opening at PeaceHealth is $18.30 – $27.46. The actual hiring rate is dependent upon several factors, including but not limited to, the job/position responsibilities, location, terms of the applicable collective bargaining agreement, education, training, work experience, seniority, work performance, etc.
Assists the Quality Coordinator with planning, organizing and implementing the Diabetes Self-Management Education & Support (DSMES) program according to Center for Medicare & Medicaid Services (CMS) credentialing agencies and Medical Nutrition Therapy for Registered Dietitian services. Administers the incoming referrals and schedules appointments for DSMT and MNT for Diabetes Educators and Dieticians across the Network.
DETAILS OF THE POSITION
- Document the behavioral goal attainment and the Patient’s program status set by the ADA in the Diabetes Flowsheet. Coordinate DSMT patients with Endocrine services. Verify orders, secure follow-up appointments.
- Perform clerical and administrative support functions including, but not limited to: referral review and scheduling for all Multi and Expansion sites throughout the network*; record-keeping; provider communication per dept standards; inventory and processing supply orders; maintaining patient records, forms revision.
- Oversee the data entry and completion of referral forms/notifications. Obtains insurance pre-authorization for patients and reauthorization of additional treatment. Ensures insurance carrier (including Medicare) documentation requirements are met and referral support documentation is filed in patient’s medical record.
- Efficiently manage correspondence with patients, physicians, specialists, insurance companies. Documents pertinent information on referral form and/or in medical record. Ensures practitioners are following standardized guidelines as recommended by evidenced based medicine. Communicates with billing staff and third party payors to assure coverage of patient care
- Assigned as Quality Coordinator Assistant for CMS approved DSMES programs.
- Assist Diabetes Care & Education Specialists (DCES) with patient behavioral goal attainment and documentation.
- Assist Quality Coordinator with quality, production and patient experience reporting for our ADA recognition annual review for all locations under the Primary Multi-site.
- Provide back-up coverage for the Diabetes, Endocrine & Metabolism Service (DEMS) in Eugene as assigned- including multi-sites and expansion sites.
- Performs other duties as assigned.
WHAT YOU BRING
- High school diploma or equivalent preferred.
- Minimum of two years of experience in a medical clinic setting performing administrative support functions (i.e. patient access, referrals, clerical duties).
- Experience in a Diabetes or Endocrinology clinic setting preferred.
- Clinical medical background or experience working in a high volume clinic with multiple complexities preferred.
OTHER SKILLS:
- Must be self-motivated and able to work both independently and in conjunction with others.
- Medical terminology familiarity
- Proficient computer skills including Microsoft Office applications.
- Strong attention to detail.
- Good oral and written communication skills.
REGION/LOCATION NOTES:
- Oregon Network Multi-sites include: Eugene, Cottage Grove, Florence and RiverBend Pediatric Endocrinology. Expansion sites include Barger Family Medicine and Creswell Family Medicine.
PHMG PHYSICAL / COGNITIVE REQUIREMENTS:
- Work requires moderate physical exertion from 34 to 65% of the time
- Ability to lift objects weighing 30 lbs. or less.
- Work is performed under normal working conditions with adequate lighting and ventilation.
- Job duties frequently require intense concentration and attention to detail (34-65% of work time)
