Essential Responsibilities

• 
  Notre Dame Home Health is seeking a part time Physical Therapist for home visits in the Metro New Orleans area, specifically in Orleans Parish and the westbank of Orleans and Jefferson Parishes.
• Applicants are responsible for making home visits for rehabilitation services to patients at their homes.
• PT responsible for admissions, evaluations, follow up visits, re-certifications, and discharges.
• PT manages the patient care in conjunction with referring physician, NP or PA.
• Flexible schedule is ideal for moms with kids or therapists needing flexibility while building a private practice.
• Pay is per visit plus mileage reimbursement.

Qualifications

• 
  Applicants must have a current Louisiana PT license in good standing, a current drivers license, CPR card, and reliable vehicle in good working order with current auto liability coverage.
• Applicants should have one year of PT clinical experience and home health experience is a plus, but we will train qualified applicants.
• Experience with adult ortho and neuro rehab is needed.
