Summary:
- Graduate of a physical therapy curriculum approved by: the Council on Medical Education and Hospitals of the AMA, the American Physical Therapy Association, or the Council on Medical Education of the Americal Physical Therapy Association.
- Minimum of 2 years experience in public health or previous home care experience desirable.
- If trained ouside the U.S., must meet one of the following: a graduate since 1928 from physical therapy curriculum approved in the country in which the curriculum was located and in which there is a member organization of the World Confederation of PT, a member of a member organization of the World Confederation for PT, and have completed 1 year of experience under the supervision of an active member of the APTA.
- Current IL Physical Therapist license
Responsibilities:
- Administers physical therapy treatments based on the physician's plan of care and in accordance with the physical therapy practice act.
- A ssures continuation of therapeutic plan following discharge by designing home exercise programs; instructing patients, families, and caregivers in home exercise programs; recommending and/or providing assistive equipment; recommending outpatient or home health follow-up programs.
- A ttends Unit meetings
- C ommunicates and coordinates patient care needs to meet patient goals as per agency policy.
- D evelops physical therapy staff by providing information; developing and conducting in-service training programs as requested by agency.
- D ocuments patient care visits at point of care and is submitted per agency policy.
- E valuates effects of physical therapy treatments by observing, noting, and evaluating patient's progress; recommending adjustments and modifications.
- H elps patient accomplish treatment plan and accept therapeutic devices by administering manual exercises; instructing, encouraging, and assisting patients in performing physical activities, ambulatory functional activities, and daily-living activities and in using assistive and supportive devices, such as crutches, canes, and prostheses.
- M aintains agency expected productivity requirements.
- M aintains professional and technical knowledge by attending educational workshops; reviewing professional publications; as per agency policy and state mandates.
- M aintains safe and clean working environment by complying with procedures, rules, and regulations.
- M eets the patient's goals and needs and provides quality care by assessing and interpreting evaluations and test results; determining physical therapy treatment plans in consultation with physicians or by prescription.
- P rotects patients and employees by adhering to infection-control policies and protocols.
- R e-evaluates therapy need at appropriate visit time, completing the 30 day reassessments as per policy.
- S upervises the PTA as per agency policy.
