We are looking for an educated, compassionate and dependable Personal Care Assistant to help support our clients with a variety of medical tasks. This position involves helping our clients transition from one care plan to another as well as providing needed everyday support. The ideal candidate for this position will have at least 2 years of experience providing long-term care and is able to help with a variety of tasks such as grooming, preparing meals, and assisting with transportation.

Responsibilities:

• Demonstrate proper infection control measures.
• Gather information from health care providers, family members and patients, and other members of the health care team to identify the patient's needs and concerns.
• Communicate with patients, families, and health care team members with compassion.
• Assist with personal hygiene and grooming, as requested by patient or family, including bathing, shaving, dressing and toileting.
• Provide for physical comfort of patient.
• Accompany patients to treatments and tests such as those ordered by physicians, dentist or other medical personnel.
• Monitor vital signs of patients and report changes to physician or nurse.
• Take notes or record observations about the patient's responses to treatment or condition for use by others on the health care team.
• Assist with moving patients to other areas within a facility.

Job Types: Full-time, Part-time

Pay: $14.00 - $18.00 per hour
