We are seeking a full-time Patient Representative/Driver for our Hudson Valley region. This position will be based out of our Monticello site and required to travel throughout the Hudson Valley region. Candidate must be flexible with their schedule as this position will require early mornings and late evenings. Must be willing to travel 50% of the time. This position requires to work some weekends if there is an event.

SUMMARY OF POSITION:
A Patient Representative/Driver is responsible for providing patient centered care and facilitating patient access to services in a pleasant, knowledgeable efficient manner, communicating with a diverse population using well-developed interpersonal skills. This staff member will be responsible for all aspects of the registration and appointment process for patients. The staff member plays an integral role in the medical care team as the access point for patients to receive care. She/he will perform job duties in compliance with systems and job specific standards. She/he must complete pre-driving checklist, fuel vehicle, drive to site location, set up van in position and stabilize, turn on generator and fill water tanks. This person must also troubleshoot mechanical problems of truck and support equipment, maintain cleanliness of interior and exterior of van including emptying trash and regulated waste, sanitizing floors, counters, sinks and restroom and must be able to exercise independent judgment and initiative to ensure safe and efficient operation of mobile unit.
ESSENTIAL FUNCTIONS:
· Drive 40-foot-long mobile service van/truck
· Guide patients to care and services
· Fill water tank, start generator or hook up electric power, monitor fill levels of waste water tanks. Troubleshoot potential problems with water system and other truck components (water pumps, hot water heater, heat and air systems, hydraulic jack system and onboard toilet and waste system)
· Communicates effectively with team members and patients all aspects of service
· Assist patients in and out of vehicle when necessary
· Use all appropriate SRH logs
· Responsible for the scheduling of appointments, fee collection, explanation of services and charges for patient services
· During inclement weather remove snow; apply salt as needed to maintain safe walkways from building and around van
· Maintain vehicle in efficient operating condition-check fuel, oil, water and tire pressure before each trip. Clean inside and outside of vehicle. Report any malfunctions as soon as they occur
· Responsible for telephone and mail contact with patients
· Monitor safety devices, fire extinguisher carbon monoxide detector
· Verifies all insurance and sets fees based on the sliding scale fee
· Perform daily cleaning of interior of van including floors, windows, counters, restroom, emptying trash and regulated trash
· Assist with daily reception activities and ensure that all processes needed for patient services are initiated
· Distributes materials to patients, including the patient satisfaction cards, patient rights and other appropriate materials
EDUCATION/EXPERIENCE:
High School Diploma or GED required
Driver’s License required
