Patient Care Coordinators are responsible for staffing patient referrals with appropriately matched agency paraprofessional staff, providing basic case management services, and are responsible for maintaining communication with office personnel, paraprofessional staff, and contract agencies.

Position Qualifications:

  • One or two years of experience in office work in a healthcare setting is preferred.
  • Must speak FLUENT English
  • Excellent telephone and communication skills

  • Good interpersonal skills
