POSITION SUMMARY: The Qualified Mental Health Professional – Community Services (QMHP-CS) is responsible for providing Targeted Case Management and Rehabilitative Services to youth with serious emotional disturbances and their families. Services may be provided in the participant’s home or appropriate community setting. The QMHP-CS will deliver case management and skills-based interventions targeted increasing the youth’s natural supports and strengths, fostering resilience and recovery. The QMHP-CS is responsible for accurately documenting services, attending Wraparound Team meetings as needed, and communicating participant progress to supervisors, team members, and the referring authority. All services are delivered in compliance with the Texas Medicaid Provider Procedures Manual, the Texas Resilience and Recovery Utilization Management Guidelines, YAP standards, and all state and federal regulations governing the delivery of service.

*Office is located in Austin. However, primary job responsibilities are in the community and not in the office.

*Services are primarily in person although minimal telehealth can be provided.

POSITION QUALIFICATIONS

EDUCATION: A Master’s or Bachelor’s degree in a human services related field, as defined by Texas Administrative Code i.e. Social Work, Psychology, Human Services, Education.

Position requires reliable transportation, valid driver’s license, clean driving record, and car insurance.
