There is a part time Nurse Practitioner (NP) in-home Health Risk Assessments job in Kimberling City, MO. The Nurse Practitioner (NP) will conduct in-home health risk assessments for a Medicare Advantage provider within Stone County, MO. The average in-home health risk assessment will take about 45 minutes and the nature of the visit may vary and could include an annual health assessment or post discharge assessment. The health risk assessment will be conducted in the homes of Medicare Advantage plan members residing in Kimberling City, MOand the surrounding Stone County, MO.Stone County, MO includes locations like Galena, Coney Island and Reeds Spring. Candidates could be scheduled in any one of these locations during an individual shift.

Responsibilities:

• Conduct annual in-home home health risk assessments for Medicare Members
• Evaluate prior and current medical history, medical conditions and medication history
• Provide education on medical conditions and medications
• Identify any gaps in care and provide recommendations for resolution
• No creation of treatment plans or prescribing of medications is required

Work Hours:

• Flexible shifts between Monday and Sunday
• Create your own schedule
• Minimum weekly or bi-weekly hourly commitment

Preferred Experience:

• 6 months of current NP experience is preferred
• Experience with Medicare aged patients is preferred, but not required

Preferred Qualifications:

• Nurse Practitioner License in MO
• Current CPR certification (BLS) through the American Heart Association
• Board Certification through AANP or ANCC

Education Requirements:

• MSN degree from an accredited school of nursing
