The Acadian Health department is looking for a dedicated candidate to hire full-time as a Mobile Healthcare Paramedic in Houston, TX. The selected candidate will primarily be responsible for the care and management of the clients enrolled in an Acadian Health Program.

RESPONSIBILITIES INCLUDE BUT ARE NOT LIMITED TO:

  • Perform physical and environmental assessments based on protocols specific to the patient.
  • Function in non-traditional settings and provide non-emergency clinical care with a focus on chronic disease management.
  • Respond to crisis calls and provide clinical care as directed by protocol and medical control.
  • Conduct environmental assessments in support of the patient's plan of care; identify and report items/actions that contradict the plan of care.
  • Provide ongoing interaction with patients in the form of telephone calls and in-person home visits.
  • Provide disease-specific education to the patient and/or caregivers in verbal and written formats.
  • Regularly communicate with multiple agencies and other members of the Acadian Health team to facilitate continuity of care objectives; ensuring the patient is receiving all available services and proper follow-up. Examples include physicians, nurse practitioners, physician assistants, hospital case managers, social services, area council on aging, and other community service organizations.
  • Maintain timely and accurate patient medical records utilizing the electronic health record software.
  • Participate in activities such as blood pressure or immunization clinics that promote Acadian Companies.

QUALIFICATIONS:

  • High School Diploma or GED
  • National and/or State certification as an Emergency Medical Technician-Paramedic determined by appropriate state requirements.
  • 3 years of Paramedic experience in a 911 system preferred
  • Current CPR and ACLS certifications established by the American Heart Association
  • Current and valid state-issued driver’s license determined by appropriate state requirements (equivalent of a Louisiana Class “D” driver’s license)
  • Bilingual in English and Spanish preferred
  • Previous trainer or instructor experience preferred
  • Previous Critical Care experience preferred
  • Effective verbal, written, and interpersonal communication skills
  • Demonstrated ability to provide effective coaching and leadership
  • Demonstrated teaching and educational facilitation skills
  • Ability to adhere to established standards for educational quality
  • Familiarity with basic computer applications and functions
  • Team-oriented and able to work effectively and efficiently with others

SCHEDULE: Days; 14 hours shifts; 2-2-3 schedule - 2 days on, 2 days off, 3 days on for the first week and 2 days off, 2 days on, 3 days off for the second week

SALARY: $60,000 - $80,000 DOE
