Position Description

Position Title: Mental Health Case Manager

Annual Salary: $48,000-$58,000

Direct Supervisor: Shelter Director

Position Summary:

This position involves providing mental health services to the residents of Life Center, as well as managing a caseload of economically disadvantaged individuals with intensive needs and working to improve their socio-economic situation through the use of case management procedures and principles.

Primary Duties and Responsibilities:

1. Complete biopsychosocial assessments on all residents at Life Center and provide recommendations to assigned case managers.

2. Facilitate weekly support and/or therapeutic groups for residents.

3. Develop relationships with mental health outpatient and inpatient facilities/programs., being the point of contact when a resident is in need of services.

4. Lead case management team with crisis interventions, including crisis assessments and facilitating calls to the crisis management team.

5. Assist in facilitating client placements/intakes including but not limited to; review of shelter rules and regulations, household supply distribution, and other crisis interventions.

6. Complete initial service assessments and case management paperwork, including service goal plans, financial analysis, case notes, social history, etc. for each client.

7. Carry a caseload of 10 and develop case management plans and individual self-sufficiency goals for each client.

8. Support and provide information for other supportive services working with clients, such as mental health and drug and alcohol practitioners, medical personnel, life skills program facilitators, etc.

9. Prevent and deescalate client confrontations.

10. Conduct client drug tests, recommending D&A services as necessary.

11. Assist clients with transportation, when needed to, programming, appointments, etc. (providing Septa Passes, bus schedules, accompaniment to public transportation, etc.).

12. Collect, forward and distribute Client Trust funds for clients.

13. Research and provide clients with apartment and housing lists.

14. Provide literature on budgeting, budgeting tips, and steps to homeownership.

15. Assist clients with housing and employment search.

16. Complete reports as assigned by Shelter Director

17. Back-up other staff as needed.

18. Other duties as assigned.

Position Objectives:

Case management

100% of all new Life Center residents will be assessed and referred to the appropriate intervention and supportive service that can assist them with their behavioral health needs.

100% of clients participating in clinical/psychoeducational group will be educated on taking care of mental health/positive thinking, anxiety, depression, stress, anger management, self-improvement, healthy relationships, learning from past mistakes, and living a healthy lifestyle.

Provide excellent supportive services to clients that will result in meeting individual client goals plans of self-sufficiency.

Develop and utilize a broad network of resources to best service clients.

Display a professional role model for clients.

Maintain accurate up to date files for all clients.

Qualifications:

This position requires a minimum of a Master’s Degree in Social Work, Human Services, Psychology or related field from an accredited college or university. Requires a minimum of five years of progressively responsible experience in the human services field, or an appropriate combination for this position.

The person in this position must demonstrate outstanding organizational skills and the ability to manage multiple priorities and concerns. This position requires an individual with outstanding clinical skills. This position requires an individual with knowledge of the Delaware County Social Services infrastructure and the ability to maintain effective working relationships with those service providers. The individual must have excellent communication skills, both oral and written, combined with strong interpersonal skills. The ability to work well with people from different cultural and socio-economic backgrounds is crucial to this position.

The physical demands described in this section are representative of those that must be met by an employee to successfully perform the essential functions of this position. While performing the duties described herein, the employee is regularly required to talk, hear, stand, walk, maneuver stairs, sit, stoop, kneel, crouch and crawl. The employee must occasionally lift and/or move up to 15 pounds. Specific vision capabilities required by this position include close vision, distance vision, peripheral vision, depth perception and ability to adjust focus.
