MDS Coordinator – Waukesha, WI

There is no routine floor work expected in this position, and partial remote work is possible.

We are conducting a search for an LPN or RN MDS Coordinator for a Skilled Nursing Facility in Waukesha, WI. There is NO routine expectation for also working on the floor here, and there is excellent corporate support to ensure you have the tools you need to succeed.

We are preferably looking for a candidate with a demonstrated history of successful MDS coordination who is comfortable working with IDT team members to ensure attainment of established goals and corporate standards. If you know the field of MDS, and are knowledgeable about PDPM, we want to hear from you.

Responsibilities:

• Create and submit accurate MDS reports in a timely manner.

• Provide leadership for the MDS Department, ensuring standards of practice are uniformly implemented in the facility.

• Stay abreast of current Medicare and Medicaid regulations, as well as prepare for upcoming changes in a proactive manner.

• Ensure staff have the necessary tools to efficiently capture and report clinical reimbursement for services rendered.

• Evaluate, sustain, and improve existing MDS procedures and practices where needed.

Qualifications Desired:

• WI RN or LPN licensure. License must be in good standing,

• 1 or more years of MDS Coordinator or Restorative nursing experience preferred.

• Excellent verbal and written communication skills, as well as a demonstrated ability to work well with diverse staff and residents.

• Case-mix expertise and ability to provide effective employee training and resource support as needed.
