The Grand Rehabilitation and Nursing Center at Great Neck is currently seeking a full time Registered Nurse for our MDS Assessor position with prior experience needed!
Description & Requirements:
The MDS Assessor is responsible for assuring the timely completion of accurate assessments and interdisciplinary care plans that meet Federal and State guidelines. You MUST have prior experience!!
Included is:
- Identifying resident acuity and needs,
- Helping to determine specific care needs, and
- Communicating needs/expectations to families and responsible parties.
The MDS covers such areas as the patient's mood, behavior patterns, cognitive ability and nutrition needs. Information from these assessments helps nursing home caretakers formulate individualized care plans that include support from social services, dieticians, rehab specialists and medical staff. MDS Assessor then implement and monitor these care plans to ensure them effectiveness. They must also make sure all strategies comply with Medicare requirements and ethical standards. Must meet the requirements of the Department of Health
