ACCA is looking for a Licensed Practical Nurse to support our Adult Day Health Center in Winder. Adult Day Health provides a therapeutic, supportive, and joyful* environment for members living with cognitive or physical impairments, as well as providing respite and support to family caregivers. Our centers are a community-based alternative to nursing home placement. For those living with Alzheimer’s and related dementia or needing assistance with activities of daily living. The LPN provides daily nursing care to ADH clients and works as a member of the care team to ensure our participants’ experience days that are fun and meaningful.

This is a full-time position, Monday through Friday, 7:30-3:30.
The LPN reports to the Adult Day Health Registered Nurse and supervises direct care staff in the provision of personal care for clients.

Essential Responsibilities
- Under supervision of the RN, provide daily nursing care to participants, including checking and recording vital signs and blood glucose levels, and reporting all abnormal findings to RN immediately.
- Coordinate personal care services for participants, including bathing, general hygiene, hair and nail care.
- Provide physical support for participants with personal care as needed by the participant. This includes bathing, toileting, general hygiene, hair and nail care, and eating.
- Administer medication to participants as ordered by physician.
- Maintain all participant records to standard set by funding sources, and self-monitor charting monthly.
- Track participant attendance and follow up as required on participant absences.
- Maintain and updated tracking sheet to ensure participants required screenings are completed within the required time frame.
- Ensure that daily record keeping is current, accurate and complete. Prepare needed nursing paperwork/forms for the following week, ensuring forms are updated to reflect current participant enrollment.
- Assist the Activity Coordinator with the planning and implementation of the person-centered, therapeutic activities program.
- Coordinate the preparation and serving of snacks, breakfast, and lunch.
- Coordinate mandatory annual trainings for ADH staff.
- Assist in maintaining a safe, clean and welcoming environment for participants and guests.
- Coordinate with nursing staff to ensure all ADH staff are aware of participants dietary needs and restrictions and ensure that all participants receive food that is appropriate.
- Evaluate the general health condition of participants and reports information to RN daily.
- Assist with safe transfers and lifting procedures. Provide physical support for participants who need assistance walking.
- Ensure that all universal precautions are being followed.
- File completed documents in participants’ charts at least monthly.
Agency Responsibilities
- Participate in and help plan ADH staff meetings and participate in ACCA staff development programs as appropriate.
- Attend ACCA staff meetings, annual meeting and other appropriate training.
- Complete all appropriate forms including timecards, travel sheets, and purchase orders within deadlines.
· Assist with planning and implementation of ACCA special events as required.
· Accept other responsibilities as may be required in performance of the job and/or as may be assigned by the ADH Director, VP/Director of Operations, and Chief Executive Officer.
Requirements
- Licensed Practical Nurse with a current Georgia license.
- Current First Aide/CPR certification.
- At least one year of experience in a health or social services field. Experience working in the aging field preferred.
- Strong communication skills, both written and verbal.
- Able to pass all mandated background checks.
- Valid Georgia Driver’s License and reliable transportation.
