Cottage Grove Place is looking for a part-time 3rd shift LPN/RN and a full-time 2p-10:30pm LPN/RN.

General Summary:

Working in accordance with established policies and procedures and under the direction of the Director of Nursing, this person is responsible for monitoring direct resident cares in the nursing facility, monitoring performance of the Certified Nursing Assistants and Oral Medication Techs providing those cares, assisting in direct resident cares as able around nurse required tasks, administering medications and treatments according to the resident’s plan of care, maintaining appropriate medical record documentation, communicating with residents and families, and assisting in response to medical emergencies in the areas of Cottage Grove Place outside of the nursing facility.

Supervisory Responsibility: Monitoring, teaching, directing, and disciplining CNAs/OMTs.

Responsibilities:

Essential Job Duties:

  • Receive, document, and implement physician orders. Accept and initiate physician orders with accuracy for content. Provide appropriate notation and performance of the orders received. Document in resident’s medical record with appropriate frequency, concision, completeness, professionalism meeting all regulatory requirements. Provide accurate, holistic assessment and treatment of each resident. Provide for residents’ rights as defined in the Resident Bill of Rights
  • Perform medication administrations per physician order with zero (0) medication errors.
  • Monitor, teach, redirect, and discipline Certified Nursing Assistants/Oral Medication Techs in the performance of their resident cares. Provide expectation of excellence, set the example, and follow-up in professional manner.
  • Admit, transfer, and discharge residents in coordination with other disciplines. Utilize information at hand to complete process of admission, transfer or discharge.
  • Demonstrate working knowledge of universal precautions and other infection control procedures. Document all pressure sores, skin tears, and infections in Infection Control Surveillance Book. Supervise staff compliance. Report breaks in technique to Human Resource Director.
  • All resident cares are delivered by licensed staff, such as call lights, etc.
  • Communicate professionally with resident, physician, family, and peers. Communicate resident condition/needs to Director of Nursing/Administration professionally and as appropriate. Always carry communication equipment and respond to calls in a timely, professional, polite, and efficient manner consistently.
  • Accept all medical emergency calls from the residents in the apartments and forward to the nursing working in that area or respond to the emergency personally if he or she is unable. The nursing facility is not to be left unattended to it is always best to find another respondent.
  • Communicates verbally and in writing all accidents, incidents, unusual occurrences and changes in condition to the appropriate nurse manager, resident’s physician and responsible party in a timely manner following established policies and procedures.
  • Provide for and ensure a clean, safe environment for resident and staff by complying with universal precautions, infection control standards.
  • Administer tuberculosis Mantoux testing to residents and staff as required. Provide first aide to staff who experience a chemical exposure or any other injury at Cottage Grove Place following the SDS manual and Workers Compensation procedures. Communicate this via VMM to all departments.
  • Provides and demands cost effective resident care.
  • Identify safety hazards and emergency situations and initiates appropriate action immediately.
  • Maintain acceptable standards of personal hygiene and complies with department dress code.
  • Comply with CGP’s Code of Conduct in all aspects of job including participation in training as offered and understands responsibility to report suspected violations.
  • Function under intense time pressure.
  • Get along with coworkers and work as a member of a team.
  • Required to fulfill mandate schedule as requested.
  • Maintain strict standards of resident confidentiality.
  • Model and support Cottage Grove Place’s expectation of being respectful, being responsible and being safe.
  • Know and abide by all policies and procedures as outlined in the Employee Handbook and in departmental policies and procedures, including but not limited to, prohibition against workplace violence in any form, including both physical and verbal abuse, prohibition against illegal discrimination and harassment, and compliance with all safety procedures.

Other Duties:

  • Follow established performance standards and performs duties according to facility policies and procedures.
  • Report to work on time and work all scheduled days. On time is defined as being on your assigned work station at the top of the hour. First shift, 6:00a.m, Second Shift, 2:00p.m., Third Shift, 10:00p.m.
  • Reliable and consistent physical attendance, in compliance with our policies, is required.
  • Contributes to a positive, healthy and safe work environment.
  • Accepts change in a positive manner.
  • Attend in-service training and education sessions, as assigned to maintain eligible status on DCW Registry and continued employment.
  • Maintain current certification.
  • Other tasks as assigned.

Qualifications:

Required:

  • Graduation from accredited Nursing program.
  • Currently licensed LPN (Licensed Practical Nurse) or RN (Registered Nurse) with the Iowa Board of Nursing.
  • Currently CPR certified.
  • Must be able to pass pre-employment physical, drug screen and criminal background screen.

Preferred:

  • Minimum two (2) years’ experience in long term care .
