We are looking for a Licensed Practical Nurse to join our team and provide quality care to our patients. The ideal candidate will have experience in long-term care, nursing home, and rehabilitation center settings, as well as primary care. The successful candidate will be responsible for providing direct patient care, administering medications, and assisting with patient assessments. We are looking for a compassionate individual who is committed to providing the highest level of care to our patients.

Responsibilities:

• Provide direct patient care in a long-term care, nursing home, or rehabilitation center setting.
• Assist with activities of daily living.
• Monitor vital signs and other patient health information.
• Administer medications and treatments as prescribed by the physician.
• Provide primary care services to patients in a long-term care, nursing home, or rehabilitation center setting.
• Assess patient needs and develop individualized plans of care.
• Document patient care activities in the medical record.
• Provide education to patients and families about health conditions and treatments.

Job Types: Full-time, Part-time

Pay: $26.00 - $28.00 per hour
