Mobile Crisis Response Social Worker (LMSW)

Alvita Care is a mission-driven Home Healthcare & Personal Care Agency with four branches and over 500 top-notch caregivers. Alvita Care is partnering with an organization to hire both full-time and part-time social workers for their mobile crisis team in the Bronx.

A respected client of Alvita Care is seeking experienced, full-time & part time LMSW’s in the Bronx.

DETAILS:

• Our Client is seeking LMSW’s to work full-time or part-time
• 3-5 days per week
• Rapid response mobile crisis team
• All patients in Bronx
• Partnered with another advocate for every encounter
• Assessment, documentation and follow up (with family, referring provider & community services) required.
• Follow up is in-person, phone and/or video with patients at least 1X per week.
• Majority of clients are referred to community services rather than the hospital
• Short term care - Work with patients for 2-4 weeks
• Permanent and/or contract positions available.
• Diver’s license and car is required
• Bilingual LMSW is a plus, but not required
• No evenings or after-hours call requirements
• Guaranteed hours / pay
• Permanent LMSW’s can get supervision hours to become LCSW
• $50 / hour pay rate

QUALIFICATIONS:

• NYS LMSW licensure.
• Minimum of 6 months experience
• Valid driver's license required
