We are currently seeking for a full time LSW Director of Social Services for our 176 bed facility. The ideal candidate for this position will be a Social Worker with experience working with behaviors in a long term care building. The position enjoys a complete benefits package and competitive wages. Starting salary is based on experience.

In this position you will be overseeing assessments, social service visits, discharge planning and care conferences. You will be in charge of completing your section on each MDS assessment and assisting with writing care plans for our residents. You will assist with behavioral interventions, psycho social visits and charting on each resident.

The position must function as a team member, team leader and ensure that work is accomplished and quality care is delivered

• The position must also manage the resources within their control and assist others in managing resources.
