Integrates clinical knowledge and informatics serving as a primary clinical coordinator for both home health and hospice- assisting all clinical staff on the use of the electronic medical record (EMR)- initially and on-going as software changes occur. Serves as a consultant to leadership in electronic medical record process redesign based on continued optimization of the software. Evaluates each new software release performing quality testing. Formulates findings in the form of recommendations. Supports software upgrades. Provides trouble-shooting of reported issues or concerns related to EMR functionality.


  • Integrates clinical knowledge and informatics.
  • Orients new hires for both home health and hospice to include RN, LPN, PT, OT, ST, MSW, Bereavement Coordinators, Chaplains, Hospice Medical Directors, Scheduling Staff and Clerical Staff (non-billing functions) with regard to the electronic medical record and home care documentation.
  • Develops written clinical preceptor materials to supplement hands on participation for staff of all clinical disciplines in Home Health and Hospice. Works collaboratively with the Epic educator to align education and software design and regulatory documentation requirements.
  • Optimizes software function - evaluates new software products, develops supporting educational materials and conducts clinical training education as new technology is incorporated for home health and hospice - all geographic locations.
  • Responsible for forms creation/editing to include designing or revising assessments for various disciplines for home health and hospice which comply with regulations. Redesigns/develops forms (using a forms editor Sequel program) for both home health and hospice, which meet all federal, state and Joint Commission requirements for documentation/practice.
  • Coordinates software upgrades to include testing/analyzing new functionality, collaborating with various disciplines to fully vet the impact to both home health and hospice, supports TSG staff during the laptop upgrade process. Redesigns educational materials as changes occur.
  • Supports service line technology expansions working closely with service line directors to explore/implement appropriate use of technology for home care/hospice service delivery to include the EMR and home monitoring. Leads new initiative efforts as driven by software or technology expansion. Coordinates new projects.
  • Acts as front line responder for any field or office based issue to trouble shoot end-user problems related to the EMR or laptop functionality. Serves as a resource and liaison for home care. Provides field support, making joint visits to observe use of laptop devices if needed. Provides ongoing trouble-shooting and problem solving as software issues occur.
  • Other duties as assigned.

What We Require:

Education: Graduate of an approved school of nursing required. Associates Degree required. Bachelors Degree preferred.

Experience: 2 years experience as a registered nurse; 1 year experience in home health preferred. Clinical experience with Home Health and Hospice preferred.

Licensure, certification, and/or registration: RN licensed in the state of Virginia. Must obtain/maintain AHA BLS-HCP.

Other Minimum Qualifications:

  • Knowledge of Medicare, Medicaid, and Joint Commission regulations for Home Health and Hospice.
  • Excellent computer skills.
  • Visual acuity.
  • Excellent verbal and written communication skills. Excellent organizational skills.
  • Ability to relate well to adult learners. Enjoys education.
  • Extensive knowledge of the Homecare Software with forms editing competency preferred.
  • Criminal History check is required for this position.
  • Must provide own transportation and travel to other Carilion work sites.
  • Must possess or quickly acquire knowledge in forms editing and software to create on-line educational modules.
