PATH is seeking candidates passionate about helping others make a positive change in their lives to join our ICMS division as an Outreach Case Manager at our Harbor UCLA office.
ABOUT THE ICMS PROGRAM
PATH's Intensive Case Management Services (ICMS) Program provides services to individuals experiencing homelessness with chronic medical conditions and co-occurring disorders and who are high utilizers or the Department of Health Services (DHS) hospital and outpatient system.
WHAT IS AN OUTREACH CASE MANAGER
As part of the ICMS Team, the Outreach Case Manager participates in daily outreach activities around the Harbor UCLA facility, providing outreach, engagement, and case management services to conduct interventions and quickly assess and link clients to long-term housing, health and mental healthcare, and any other supportive services necessary to their journey to obtaining and maintaining sustainable housing. Position Responsibilities include:
- Conduct daily street outreach to develop relationships and build rapport with clients to assist them with finding a home.
- Respond within specific contract timeframes to requests for street outreach services and send follow-up information to the requestor and supervisor.
- Conduct VI-SPDAT (Vulnerability Index- Service Prioritization Decisions Assistance Tool) on all new clients to identify and connect them with proper intervention and services; document results, complete match initiation form and submit all paperwork to CES coordinator.
- Collaborate with clients and the street outreach team to develop an Individualized Service Plan (ISP) for each client that addresses short- and long-term goals.
- Provide street-based case management services that connect clients to long-term supportive services and help improve their quality of life.
WHAT YOU BRING
We're looking for candidates that:
- Demonstrated knowledge and experience with advanced and evidence-based interventions including; Engagement and
rapport building with hard-to-reach clients; Harm Reduction and Motivational Interviewing strategies; Housing First;
Non-violent crisis intervention techniques; Case management interventions proven effective with vulnerable
populations. - Ability to work effectively independently and as part of a team.
- Ability to exercise mature and professional judgement.
- Strong problem-solving and conflict resolution skills.
- Strong written and verbal communication skills.
PREFERRED QUALIFICATIONS
- Two (2) years of experience in outreach, healthcare, case management, homeless services or working with
vulnerable populations. - Lived experience with homelessness and/or issues related to homelessness and experience in street
outreach
MINIMUM QUALIFICATIONS
- (Two) 2 years of related experience or educational equivalent
MANDATORY REQUIREMENTS
Regardless of qualifications, candidates must:
- Have employment eligibility verification
- Have or be able and willing to obtain CPR/First Aid training
- Provide proof of full COVID-19 vaccination
- Successfully complete the following as a condition of hire:
- Tuberculosis Test
- Background Screening
- Drug Test
DRIVING
Driving is an essential function of this position. To meet the employment requirements for this role, all candidates must:
- Have reliable transportation
- A valid driver's license
- Proof of insurance and ownership for personal vehicles used during work duties
- The ability to qualify for PATH's insurance coverage
COVID-19 VACCINATION
Effective 11/01/2021 ll require all employees to be fully vaccinated for COVID-19. All prospective hires will be expected to provide proof of full vaccination as part of the pre-employment credentialing process.
