Hospice On-Call Registered Nurse (Sign-On Bonus Paid Upon Hire)
Job description
The On Call Nurse responds to all agency calls outside of normal business hours. Works collaboratively with the On Call Coordinator to respond to calls requiring immediate nursing expertise. **Schedule needed is 7-on, 7 off (full time). Or weekend daytime visits (Saturday and Sunday part time - no sign on bonus for part time)
ESSENTIAL DUTIES AND RESPONSIBILITIES include the following:
**After Hours Nursing visits and Admissions
- Works collaboratively with On Call Coordinator, RN Clinical Manager and staff to assure all patient care concerns are addressed during non-business hours.
- Expectation: Responds timely to request from on call coordinator for assistance with patient concerns.
- Completes activity log/on call report in detail for all calls received during hours of work and forwards to designated staff at end of shift.
- Expectation: All incoming and outgoing calls are recorded on the on call log accurately and timely.
- Answers all calls promptly, identifies concerns and responds appropriately, refers any unresolved issues to administrator on call.
- Expectation: Administrator on call will be notified of complaints, patient or employee injury, monetary or physical damage and any requests from patients or families to speak to administration.
- Communicates clearly and directly with other staff members and reports any problems to the assistant or executive administrator.
- Expectation: Communication is ongoing, clear, concise and complete.
- Acts as a clinical resource to On Call Coordinator and staff.
- Expectation: On call nurse will respond timely to patient’s need for clinical evaluation or communication with field staff.
- Completes activity log/on call report or in a case management note, in detail for all calls received during hours of work and forwards to designated staff at end of shift.
- Expectation: All assigned work is completed thoroughly and accurately within designated time frames. All incoming and outgoing calls are recorded in the log including time and summary of calls. On call nurse will document in the patient medical record under case management notes, any conversations related to changes in the care plan or other noteworthy information that needs to be available to all team members.
- Actively participates in quality assurance and performance improvement teams and activities.

Requirements
Education: Graduate of an accredited School of Nursing.
License: Current Pennsylvania Nursing license.
Experience: Five (5) years recent clinical Home Health experience preferred.
Satisfactory Criminal Record Clearance
- Effective communication skills, and Proficient in English
- Excellent organizational and time management skills including timely charting
- Familiarity with professional and technical emerging knowledge
- Problem-solving skills and ability to multi-task
- Compassionate with teamwork skills
- Current license, certification, or registration
- Covid-19 Vaccination or willingness to obtain Covid-19 vaccination
- Committed to safety precautions and high ethical standards
- Sound judgment and attention to detail
- Emotional/mental stability and stamina
- Formal nurse training
- Current licensure in state
- Must be a licensed driver with an automobile that is insured in accordance with state and/or organization requirements and is in good working order
- Prolonged or considerable walking or standing. Able to lift, position, and/or transfer patients. Able to lift supplied and equipment. Considerable reaching, stooping, bending, kneeling and/or crouching. Visual acuity and hearing to perform required nursing skills
Knowledge of:
- Operation of standard office equipment
- Microsoft Office software packages, Email
- County, State and Federal Regulations
Ability to:
- Demonstrate customer service, detail oriented, critical thinking, problem analysis/judgement and decision making skills
- Communicate effectively, verbally and in writing
- Establish and maintain effective working relationships with coworkers, patients and their families and the public
