We are currently seeking a Hospice Coordinator to join our team, acting as the link between our interdisciplinary team members and ensuring that patients receive the quality care they deserve.

Responsibilities:

- Coordinate with interdisciplinary hospice team members, including medical staff, social workers, and volunteers, to provide comprehensive patient care
- Manage patient schedules, ensuring timely and efficient delivery of services
- Maintain accurate and up-to-date patient records, care plans, and documentation
- Communicate with patients and their families, addressing any concerns or questions they may have
- Assist with the recruitment, training, and scheduling of volunteers
- Ensure regulatory compliance and maintain familiarity with hospice policies and procedures

Requirements:

- Bachelor's degree in healthcare administration, business administration, or related field preferred
- Minimum of 2 years of experience in a hospice or healthcare setting, with a focus on care coordination or case management
- Strong organizational, communication, and problem-solving skills
- Ability to multitask and remain professional in a fast-paced environment
- High level of compassion and empathy for patients and their families
- Proficiency in electronic health records and standard office software

Job Type: Full-time

Pay: $22.00 - $28.00 per hour
