Penn Medicine at Home (PMAH) is hiring an experienced Hospice Facility Education Coordinator to service patients in the following areas: Bucks County, Chester County, Delaware County, Montgomery County and Philadelphia County. This is a great opportunity for a Hospice Facility Education Coordinator to truly become a partner in patient care and work at the top of their license, while impacting their community.

Summary:
Reporting directly to the Hospice System Director in collaboration with the Hospice Professional Development Specialist, the Hospice Facility Education Coordinator will provide clinical education to support end of life outcomes for patients residing in long term care facilities. The Coordinator will develop innovative programs for facility staff to ensure clinical competence, collaboration and compliance with end of life hospice care. The Coordinator will be responsible for outreach to the facility clinical leadership team in collaboration with the hospice care team to identify educational needs for symptom management, psychosocial and spiritual support, bereavement care and other clinical services. The Coordinator will provide formal and informal education programs, operational/compliance updates, provide information to patients and families on hospice care, and supports the provision of exemplary interdisciplinary care. The Coordinator will support community relations activities, bereavement support for staff, ongoing education, and other activities related to care provided by Penn Medicine Hospice to generate positive name recognition and consumer relations.
Responsibilities:
Ensures customer satisfaction through complete and continuous measurement of satisfaction levels for all key customer groups. Completes and maintains the Operational Information sheets for the clinical teams.- Works with Clinical Operations, UPHS Partners, Penn Medicine Hospice to develop programs, services and change initiatives that anticipate hospice facility staff needs and builds symptom management expertise among facility staff. Participates in positioning PM Hospice as a leader in care in the area.
- Provides educational activities and supports facility staff in activities that promote excellent patient care such as goals of care conversation competency, end of life care and symptom management.
- Anticipates facility staff needs and assures that they are accommodated. Identifies individual patient's/customer's special and diverse needs and modifies interactions to satisfy them.
- Delivers internal and external customer service seamlessly across system boundaries. Takes action to make up for lapses in service. Ensures patient/customer/family confidentiality and respect.
- Develops and maintains a high level of knowledge and expertise in Hospice services. Complies with identified service standards. Provides accurate information about Penn Medicine Hospice regarding criteria for admission, available services, reimbursement criteria, and coverage under Medicare and Medicaid. Troubleshoots service failures. Manages up to the appropriate leader.
- Develops and implements outreach and educational strategies with facilities to promote high quality, clinically excellent care.
- Maintains communication with referral sources and continually monitors satisfaction. Conducts periodic meetings with facility staff and hospice teams to provide data.
- Acts as a resource to clinical staff and referral sources for hospice and end of life care.

Registered Nurse - PA (Required)- Driver's License (Required)

Education or Equivalent Experience:
Bachelor of Science Nursing (Required)- 4+ years relevant experience
