• Start of Care (SOC) $140 (+mileage)
• PT Visit $110 (+mileage)
• PT Evaluation $125 (+mileage)

• 40 hours per week for 5 days
• Willing to see patients in East Bay, California

• Minimum of 3 days, 5 visits per day

• Graduate of a Physical Therapy program.
• Has a current license as a Physical Therapist in the State of California
• With Home Health care experience for 6 months (for Per Diem).
• CPR certification
• Demonstrates ability to work independently with minimal supervision.

• Must have a valid driver’s license, and auto insurance.
• Required to present a negative TB test and other tests as required by agency policy
• Must be able to do field work
• Must have an excellent written and verbal skills
• Excellent planning, organization, and presentation skills
• Ability to think and work independently and with direction and communicate with staff members in a fast-paced and sometimes stressful environment.
• Must be in good health and be able to fulfill the physical requirement of the job.
• Must be able to lift a minimum of 25lbs.
• MUST BE WILLING TO SEE PATIENTS IN East Bay
