HIV and Addictions Nurse – Mason Square SIGN ON BONUS

The HIV Ambulatory Nurse collaboratively leads the development and delivery of high quality patient care including coordinating care and services for patients with Hepatitis C, HIV infectious disease and or addictions.

Will function as a community liaison and will be expected to have the capacity to offer the following components of medical case management: medical care coordination, phone check-in, social services coordination, adherence support, HIV/STD/HCV risk reduction counseling, addiction services, benefits counseling, coordination of home delivered meals if required, as well as coordination of other services as needed (e.g., transportation, peer support, housing, etc).

Required Work Experience:

• Massachusetts license as an RN and BLS CPR certified.
• Minimum of 3 years of nursing experience
• Preferred candidates will have knowledge of HIV addictions and or infectious disease, have clinical nursing experience as well as care management for similar patient population
• Experience providing clinical care, education, and treatment to patients with substance abuse diagnoses
• Demonstrated interest to work with patients from diverse economic and cultural backgrounds
• Excellent verbal and written communication and interpersonal skills.
• Must have competency in delivering exceptional nursing care and have proven success in partnering with both physicians and staff

Bachelor of Nursing Degree, Diploma in Nursing (Required)

Certifications:
