The Health Navigator may function at the initial entry point of the care continuum, providing outreach, case finding, peer support, and information about the health care system, at care transition points between health care agencies such as from emergency department/inpatient unit to community health center, providing linkage and tracking, or in ongoing care, providing health education/health promotion, in partnership with other members of the Behavioral Health Integrated Team. Integrated behavioral health care is the systematic coordination of physical and behavioral health care. The idea is that physical and behavioral health problems often occur at the same time. Integrating services to treat both will yield the best results and be the most acceptable and effective approach for those being served.

This position can be hourly or FFS!

POSITION REQUIREMENTS:

  • Provide outreach to homeless or at risk individuals in area shelters, on the streets and at other locations.
  • Engage homeless individuals and assess eligibility for the PATH program and their strengths, challenges and obstacles to housing and mainstream resources.
  • Arrange for clinical assessment to determine eligibility for the PATH program.
  • Transition participants to local community resources.
  • Provide, in coordination with Peer Navigators, short-term Case Management support for Participants for up to six (6) months.
  • Develop a service plan with participants of needed supports for stabilization and community integration.
  • Assist participants in completing the disability determination process and attaining for MaineCare and SSI/SSDI benefits.
  • Assist participants in attaining mainstream resources, housing, healthcare, treatment, employment, education resources.
  • Coordinate with local Housing Navigators and Local Administrative Agency’s (LAAs) to assist participants in attaining a housing resource.
  • Work as a team with Peer Navigators and collaborate with other service PATH Providers in the area.
  • Maintains administrative responsibility (charting/documentation, etc) for assigned caseload.

EDUCATION:
Bachelor's Degree required and must be able to obtain provisional MHRT/C certification.
