Primary Purpose:
Responsible for developing a relationship with members and assisting the member through the disease care coordination and management process by providing support, encouragement, and education on their chronic condition. Support coordination of care and condition management activities with members, caregivers, and providers.
Minimum Specifications:
Education
- Associate degree in a health- related field and / or LPN / LVN is acceptable
- Current health coach certification from an accredited professional health coach training program in health promotion, chronic condition management, or health education preferred job
Experience
- Minimum 2 years experience working with members with chronic conditions or disease management and health education activities
Equivalent Education and/or Experience
- May have equivalent combination of education and /or experience in lieu of education
Certification/Registration/Licensure
- Chronic condition management or health education and chronic care professional certification, NSHC CHC preferred
- Current valid LVN /LPN license from the Texas Board of Nursing or a valid compact LVN license
Skills or Special Abilities
- Professional verbal and written communication skills with internal and external audiences
- Strong member advocate: willing to go above and beyond normal responsibilities to provide the best service possible
- Ability to assist member in navigating the healthcare system and community based resources
- Culturally sensitive and competent for assigned membership
- Strong organizational skills, ability to multi-task
- Ability to determine when to escalate issues appropriately and in a timely manner
- Proficient computer skills - Bilingual preferred (Spanish)
Responsibilities
1. Develop trusting relationship with members to support the member through the process of changing behavior and improving management of their health using motivational interviewing techniques and change management concepts
2. Assist in coordinating member's relationship with multiple service systems which may include obtaining a medical home, instructing on appropriate use of the medical home, assisting member with overcoming transportation barriers, informing member about available community resources and the use of those resources
3. Support and coach members to improve the management of their chronic condition including health education in a culturally appropriate format, social support to improve the members adherence to the medical treatment plan, encouraging the member in the adoption of habits that are conducive to improved management of their chronic condition, development of an individualized plan including goals and plan of action, contacting and engaging members primary care givers
4. Serve as a resource to the Service Coordination team member
5. May conduct in person visits with the members to support member engagement, care plan goals and outcomes
6. May perform outreach activities to assist in locating members and engaging in service coordination services
7. Maintain current and accurate documentation confidentially according to HIPAA regulations
