About Jericho Project

Jericho Project is a nationally-acclaimed nonprofit ending homelessness at its roots by enabling homeless individuals and families to attain quality housing, employment and mental and physical health services. The 40-year-old nonprofit serves over 2,500 individuals, including 700 veterans, annually. Jericho has been a key partner in New York
City’s initiative to end veterans’ homelessness, and is leading bold, innovative strategies to do the same for families and young adults.

Our programs touch four cornerstones of a person’s life: housing, employment, wellness and family stability. We create a culture of “moving on” through a foundation of housing and supportive services provided by expert program specialists and case managers.

We have over 550 units of supportive housing including 9 residences in the Bronx, Harlem, and Astoria, as well as scatter-site apartments throughout NYC.

About the Position

Jericho Project is seeking to recruit a full-time Health Care Coordinator, to develop and implement new health-care related services across the agency. The individual would be a community Registered Nurse (R.N.) with experience working with vulnerable populations.

The overall purpose of the position is to reduce unnecessary in-patient hospitalizations, emergency room use and improve the health care outcomes of Jericho Project service recipients through education, introduction of best practices and consultation to staff, rather than through direct practice.

Responsibilities:

  • Develop trainings for staff and clients on health related matters including:

Hypertension, Diabetes, Liver Disorders, Lifestyle and Risk factors, Diet and
Exercise, Health Literacy.

  • Develop, monitor and trend health related performance metrics.
  • Lead quality improvement groups for developing methodologies to improve health care metrics.
  • Develop collaborative relationships with healthcare settings for improved referral process and patient care communications.
  • Consult with staff regarding individual tenant health care issues, providing education as to conditions, potential treatment and resources for further education.
  • Consult with staff to develop individualized, client centered goals relating to tenant’s healthcare.
  • Review In-patient hospitalization and Emergency Room discharge instructions to assist staff in understanding follow-up needs post treatment.
  • Review tenant physical exams to assist Case Managers in developing health related plans of service.
  • Assist in coordinating telehealth to increase communication with health professionals.
  • Assist in development of Aging-In Place Initiative.
  • Develop and coach staff on conducting wellness related activities for tenants.

Requirements:

  • BSN, RN
  • 5 years community nursing experience.
  • Background as a Psychiatric Nurse preferred.
  • Experience with vulnerable populations required.
  • Bilingual Spanish/English is a plus.

Compensation:
The salary range for this position is $80,000.00 annualized . Jericho Project offers a comprehensive benefits package.
