Adelphoi’s Adoption program has a full-time Permanency Specialist position available at the Towanda location. Services will be provided to Clients and Families in the surrounding counties. All SWAN services will be provided.

Salary range from $40,000 to $45,000 based on experience.

Services delivered will include all SWAN services (Child Specific Recruitment, Child Preparation, Child Profiles, Family Approval documents, matching, finalization, and Post Permanency). This position will coordinate and implement permanency services with clients, biological family members, foster/kinship families, and adoptive families within the context of a foster or kinship home, group home, or residential treatment facility setting.

Duties Include:

• Provide regular sessions with clients and families in order to implement SWAN services
• Conduct interviews with the child, county, birth family, foster family, etc.
• Conduct a thorough review of county files and collaborate with county workers about identified client
• Conduct sessions utilizing Child Preparation activities and development of lifebooks
• Document required information both written and electronically (i.e., medical and daily logs, session notes etc.) and completes required reports in timely manner
• Maintains daily entries for service work completed
• Maintain a client and or family file for services delivered
• Act as an appropriate role model for youth at all times
• Travel extensively to clients' home, counties, etc. in order to perform job duties, as well as to attend meetings, training and/or seminars as required
• Flexible hours: schedules sessions when both youth and families are available. Ability to work a Hybrid Schedule (Mix of Office and Home hours)

Requirements:

Master’s degree preferred, Bachelor’s degree required. Degree in Psychology, social work, or related field preferred and 2 years direct care experience.
