The community you’ll be caring for:

Zephyrhills North

• Located in the historic town of Zephyrhills with easy access to Tampa, Wesley Chapel, and Lakeland
• 113 bed recently renovated skilled nursing facility
• In close proximity to Adventhealth Zephyrhills

The role you’ll contribute:

The Social Worker functions on behalf of the nursing home and the residents by promoting the general well-being of each resident through identification of their emotional and psychosocial needs. The Discharge coordinator is responsible for implementing patient discharge plans, Interacts with representatives from insurance companies, managed care organizations, home care agencies, skilled nursing facilities, durable medical equipment companies, and transportation agencies as required. Responsible for assisting in coordinating care and services for assigned patients and in collaboration with the IDT members residents and legal responsible party. Ensures discharge arrangements are in compliance with payer requirements, and regulatory guidelines.

• Provides medical related social services so that each resident may attain or maintain the highest practical level of mental, physical, and psychological well-being.
• Ensures that all PASRRs are completed accurately and timely in coordination with the admissions department.
• Stays current on Federal and State regulations pertaining to social services.
• Assist the resident, responsible party, and supportive others in the adjustment and orientation to the nursing home after admission.
• Conducts psycho-social assessments of residents.

Qualifications

The expertise and experiences you’ll need to succeed:

• Bachelor’s Degree in Social Work or a Human Services field including but not limited to Sociology, Special Education, Rehabilitation Counseling, and Psychology or additional experience as a Social Worker in a skilled nursing facility.
• In lieu of required education, minimum of 5 years’ current experience as a Social Worker in a skilled nursing facility.
• Must be 18 years of age or older
