ActivCare operates specialized senior living communities in Orange County built to serve the changing needs of those with memory loss by offering different levels of compassionate memory care.

ActivCare is expanding our mission to enhance the quality of life for those with memory loss and embrace an underserved population affected by dementia – individuals with Down syndrome.

We are seeking a Direct Support Professional for Seniors with Intellectual and Developmental Disabilities who are affected by dementia.

JOB OBJECTIVE:

The Directo Support Professional will work directly with our residents with Down syndrome living in ActivCare Orange and ActivCare Yorba Linda. The Direct Support Professional is responsible for the overall care and services to our specific residential population and to enhance the quality of life for those individuals with intellectual and developmental disabilities who are affected by dementia. The Direct Support Professional will report directly to the Executive Director.

Direct Support Professional / House Manager Responsibilities:

• Be a specialist with an emphasis on individuals with Down syndrome affected by dementia.
• Learn our new process and be able to take ownership of this new program.
• Proved direction, input, and feedback.
• Identify strengths, develop opportunities, and implement progress and/or improvement plans.
• Works with Program Leader and other staff to ensure proper oversight of residents is being carried out.
• Responsible for health and safety of residents.
• Prepare Individual Program Plan for each resident, in cooperation with the individual and other staff; and attend all IPP meetings with each resident.
• Provide supervision, instruction, and assistance for residents as required in the Individual Program Plan or as needed.
• Maintain and/or cause to be maintained, all records (IPP Program, financial, medical, etc.) in accordance with regulations daily.
• Conducts audits and walkthroughs in assigned residents’ units to ensure compliance with Title 17 and Title 22.
• Facilitate individual and group outings and delegate responsibilities as needed.
• Management of resident’s funds, submit P&I cash requests.
• Work with Nurse and other Care Providers to ensure residents’ medical needs are met & in compliance with Title 17 and Title 22 regulations.
• Manage purchases and/or order forms to appropriate persons for: food, household supplies, paper goods, etc.
• Maintain family/conservator contacts.
• Attend Residential Support Manager and Staff Meetings.
• Other duties as assigned.

Education, skills, & abilities:

• Previous experience as a direct support professional working with adults with disabilities or related field.
• Have or can obtain a CPR/First Aid Certification.
• Working knowledge of people with intellectual and developmental disabilities and their families.
• Ability to appear for work on time, follow directions from a supervisor, interact effectively with co-workers, understand, and follow posted work rules and procedures, and accept constructive criticism.
• Must be able to pass state and federal level background check and Live Scan fingerprinting.

Working Conditions:

• This position will work at two different residential memory care communities in Orange County, ActivCare Orange and ActivCare Yorba Linda.
• Works primarily in a setting involving standing, sitting, bending, stooping and/ or crawling.
• This position requires using PPE when performing certain duties.
• May come in contact with bodily fluids which could potentially contain blood borne pathogens.
• Frequent lifting of up to 50lbs is required.

Pay: $19.00 - $21.00 per hour
