Essential Duties:

• Promotes positive customer service culture which encourages quality patient outcomes and promotes staff retention
• Assists in performing comprehensive psychosocial assessments with patients in order to determine appropriate level of care and discharge planning needs
• Conducts informational/educational sessions providing appropriate materials to educate patients on addiction recovery and aftercare options
• Maintains current knowledge of resources available within the community, maintains supply of resource materials to be distributed to patients when needed
• Completes appropriate forms and contacts all appropriate individuals to facilitate the transfer/linkage of patients to other community-based agencies and services
• Ensures all referrals are linked through follow-up
• Documents all relevant clinical information pertaining to patients based on assessment, education, and discharge planning activity
• Prioritizes and organizes work according to the acuteness of the situations presented
• Maintains good working relationships with Kankakee County law enforcement and staff
• Attends in-house (FGC) and off-site trainings for continuing education, in order to expand knowledge, improve the quality of patient services, and to maintain licensure and/or certification at all times
• Other duties as assigned

Work Schedule: Monday - Friday (9:00a - 5:30p)
