POSITION SUMMARY:


DC Primary Care Association’s (DCPCA) member health centers provide comprehensive primary care to 1 in 4 Washington D.C. residents, reaching a total of over 1 million visits a year. DCPCA’s Programs and Health Center Support team provides member health center staff with the training and tools to strengthen and improve access to care, operational excellence, and health outcomes. Our member health centers provide high-quality, efficient, comprehensive, affordable, culturally competent medical, dental, and behavioral health care.


The Program Coordinator is part of an innovative and growing department at DCPCA, supporting the improvement of clinical and patient outcomes through implementation of evidence-based services. The Program Coordinator provides trainings, technical assistance, and collaborates with other organizations to optimize clinical practice and improve quality performance.


DCPCA’s Programs and Health Center Support department supports health centers in providing high-quality care through technical assistance, face-to-face trainings, webinars, dissemination of resources, and peer-to-peer connections. Our work is data-driven, evidence-based, and grounded in QI principles. We seek efficient and proven solutions to systemic healthcare obstacles for vulnerable populations. We leverage this knowledge to connect resources and best practices to health center staff and patients. Our Programs and Health Center Support department provides training and technical assistance to health center clinical and administrative staff on a range of care topics including:

  • Patient Centered Medical Home (PCMH), team-based care and care coordination
  • Chronic disease prevention and management, in particular diabetes and cardiovascular disease
  • Cancer screening (colorectal, breast, and cervical)
  • Behavioral health integration
  • Substance use disorder treatment, including opioid abuse
  • Oral health and oral health integration
  • Special populations including: individuals dealing with housing insecurity or homelessness, and/or individuals of the LGBTQIA+
  • Social Determinants of Health (SDOH)
  • Trauma-Informed Care
  • Quality measure collection, analysis, and reporting
  • Billing, coding, and documentation
  • Best practices for use of available EHR, HIT, and HIE tools, including Meaningful Use (MU)
  • Workforce development
  • Emergency preparedness

The Program Coordinator will support our emergency preparedness, (behavioral health, substance use and tobacco programs XXX List other areas here) managing a set of projects that focus on providing training, technical assistance, and learning collaboratives to advance outcomes and equity for chronic disease. In addition to providing training and technical support to health centers, the QI Coordinator will work with a variety of local and national partners to advance systems-level solutions to emergency preparedness, (behavioral health, substance use and tobacco management and care in the District.


The Program Coordinator will have expertise and a passion for health care and public health. The ideal candidate brings a broad background of experience in healthcare or public health with direct experience in primary care settings. This talented professional has strong analytical and data analysis skills, with the ability to balance multiple priorities and creatively use available resources to drive transformation. Excellent training and meeting-facilitation skills are a must with the ability to build relationships with diverse stakeholders. This professional will possess a customer-service orientation, with enthusiasm for finding solutions and connecting resources in support of health center goals. ESSENTIAL DUTIES AND RESPONSIBILITIES:


Technical Support and Training to Health Centers

  • Plan, coordinate, facilitate, and evaluate learning sessions/trainings for health center staff in collaboration with project partners including curriculum design, agenda setting, and trainer identification.
  • Serve as a resource for members on key content areas – connecting health center staff with the tools and resources they need to reach their care goals.
  • Provide technical assistance to member health centers around identified needs.
  • Work intensively with health center teams participating in assigned projects to refine delivery system and implement best practice concepts.
  • Design, develop, and implement member assessments around clinical performance and TA needs to identify opportunities for improvements and possible solutions.
  • Lead in the coordination of health center Peer Groups, including planning meeting logistics, communicating with members, preparing content for meetings, and directing follow-up coordination.

Knowledge Contribution and Partnerships

  • Work with local and national agencies, independent subject matter experts, and member clinics to identify best practices and evidence-based practices.
  • Conduct research to expand the QI department’s subject matter expertise.
  • Support the development of member performance feedback report synthesis, analysis, and dissemination to member health centers.
  • Aggregate and analyze qualitative and quantitative data in support of project activities as well as for reporting purposes to participating health centers, funders, and DCPCA staff.
  • Disseminate resources to health center staff and regional bodies in pursuit of project-specific goals.
  • Prepare project reports, memos and maintain project records and statistical information.
  • Support administration of program grants and deliverables including coordination of project activities, tracking deadlines, reporting activities, and convening the stakeholders needed to ensure that projects and products are of the highest quality.
  • Attend relevant stakeholder and advisory meetings to support overall program implementation and evaluation.
  • Other duties as assigned.

QUALIFICATIONS, KNOWLEDGE & SKILL REQUIRED:


Required qualifications and experience:

  • Master’s degree or Bachelor’s degree in relevant field with 1-3 years of relevant work experience equivalent
  • Experience with community health care and primary care
  • Experience with quality improvement measurement
  • Experience with various QI principles and approaches (e.g., Model for Improvement, Breakthrough Model, Lean)
  • Knowledge of several subject matters of focus for the QI Department (see above summary).
  • Excellent meeting-facilitation skills
  • Proven ability to teach others and conduct trainings for adult audiences
  • Excellent written and oral communication skills
  • Eager and driven to stay up to-date on the latest science and innovations regarding primary care and quality improvement
  • Proven experience with project management, in particular demonstration of ability to organize various activities and goals, manage multiple projects simultaneously, plan strategically, and see projects through to completion
  • Strong relationship-building skills, with proven ability to develop effective relationships with diverse stakeholders (government, community, and business environments)
  • Ability to work collaboratively with physicians, staff and external organizations
  • Ability to work effectively with diverse teams as well as demonstrated awareness and value of cultural competence
  • Team-player, ability to coordinate work collaboratively and resolve conflicts in a precise and timely manner
  • Effective analytical and problem-solving skills, with the ability to quickly gather information (quantitative and qualitative), identify trends, and propose solutions
  • High degree of proficiency with MS Office products, including Word, Excel, PowerPoint, and Outlook. Experience with Visio, Tableau or Access experience a plus
  • Familiarity with functions of an electronic health records (EHR) software

Preferred qualifications:

  • Experience in or with a Federally Qualified Health Center (FQHC)
  • Experience with eClinicalWorks (eCW)

ADA SPECIFICATIONS:


This position will be based in an office in Washington D.C. but will require travel to health centers throughout D.C. This position is primarily sedentary and may require some bending and lifting.
