Position Summary
The Discharge Coordinator has knowledge of contracts, contracted venders and criteria for post-discharge medical and other clinical needs. The Discharge Coordinator implements discharge planning referral orders in constant collaboration with inpatient case management. The Discharge Coordinator promotes safe and timely discharges following standardized processes and decision trees to assure hand off of the information needed by the post-acute provider. The Discharge Coordinator must maintain timely, accurate and ongoing documentation to ensure the patients post-acute care needs are well coordinated.
Essential Functions and Responsibilities of the Job
1. Actively collaborates with case manager, social workers and discharge planners to facilitate a timely and accurate response to the discharge planning referrals. Facilitates communication to ensure that the patient post acute care needs are coordinated.
2. Follows standardized processes and decision trees for referrals for ordered post acute care needs including but not limited to DME, home health SNF, subacute, LTACH.
3. Referrals are timely and accurate and are per contracts, contracted venders and department processes using standard work and decision trees and vender data base.
4. Serves as an informational resource for post acute care contracts, vendor contracts and acceptance criteria. Maintains and updates data bases so the information is easily found by others.
5. Documentation in the medical record, WQ and data base is per timeliness requirement, provides the information required and is updated as changes occur.
6. Performs other duties as assigned.
Pay Range: $27.52 hr – 39.90 hr
Placement in the pay range is based on multiple factors including, but not limited to, relevant years of experience and qualifications. In addition to base pay, there may be additional compensation available for this role, including but not limited to, shift differentials, extra shift incentives, and bonus opportunities. Experience
- Must have 4+ years of prior relevant work experience i.e.. previous discharge planning experience, community resource development, post acute admissions coordinator, medical group care coordinator etc.
Education
- Prefer LVN or higher degree in a healthcare related field.
