We have an opening for Community Outreach Coordinator.This is a Part Time position consisting of 28 hours per week.

DESCRIPTION OF POSITION

Develops, manages, and evaluates internal and external wellness programs and related services, and the implements community health related initiatives sponsored by Skyline Health. Conducts outreach and endorses the hospital and its programs in the community to help carry out Skyline’s mission of promoting an exceptional level of health and well-being in our community. Ability to manage and grow Skyline Health’s volunteer program.

EDUCATION BACKGROUND AND EXPERIENCE

· Bachelor’s degree in a health care related field is preferred, or equivalent prior experience.

· Knowledge of theories and principles of community health, and community health promotion and education.

· Two or more years’ experience in health promotion and programs.

· Experience in developing partnerships with local organizations and service providers.

· Experience in managing a volunteer program.

· Ability to work independently and as a member of a multi-disciplinary team.

REQUIRED KNOWLEDGE AND SKILLS

· Supports Skyline Health mission and strategic vision.

· Manages hospital resources appropriately.

· Complies with hospital policies and programs.

· Maintains skill levels, licenses, and certifications.

· Complies with all state and federal Compliance and HIPAA requirements.

· Keeps lines of communication open between self and other hospital personnel.

· Competent with computer programs.

· Competent in social media communications.

· Proper telephone etiquette and appropriate personal interaction.

· Shows professional respect and courtesy to patients, families, physicians or colleague.

· Ability to work independently and to work well with others.

ESSENTIAL JOB FUNCTIONS

· Identify opportunities for Skyline’s community health and wellness program.

· Create, implement and analysis Skyline Health’s internal wellness program.

· Work with key stakeholders to develop and coordinate wellness and health and education programs.

· Work closely with marketing staff to share Skyline Health’s program information, partner information and health promotion and education via a variety of communications channels.

· Connect and build relationships with a broad range of community members.

· Represent Skyline Health on a variety of community/regional committees and organizations.

· Be visible in the community by participating in community events and activities.

· Create positive working relationships with other organizations, businesses and the community.

· Manage and grow volunteer program.

· In conjunction with HR Manager, manage and communicate Employee Assistance Program.

· In conjunction with Marketing Manager, maintains website and produces monthly employee newsletter.

· Use judgment and problem solving skills to resolve issues with health and wellness programs and activities.

· Participate on behalf of Skyline Health in the Community Health Needs Assessment, communicate results and develop improvement plan.

· The position requires some travel, and potential for evening and weekend meeting, events or activities.

· Completes other duties as required.
