Job description
National Council on Aging Vaccine Uptake Project:
· Be knowledgeable about and have experience working in the Chicago’s south and southwest side communities as well as the suburban communities of Cicero, Berwyn, Stickney and others with significant populations of Chinese and Latino older adults to, to provide vaccine education and address vaccine hesitancy in the community’s older adult population.
· Reaching older adults who are unvaccinated or who have not yet received a booster shot via one-on-one contacts, health fairs, presentations and other community outreach.
· Work with Alivio’s Seniors and Benefits Enrollment, and CHWs team will address the individual motivations and barriers. In talking to people on an individual level, those conversations will include:
o Motivational interviewing to address barriers to vaccination,
o Connecting clients to appropriate vaccination/booster delivery whether that is receiving COVID-19 and flu vaccines at Alivio vaccine sites operated by Alivio, finding a local site, arranging transportation to a site or scheduling in-home vaccinations,
o Ensure the use of appropriate language, cultural competency, relatability, accurate information, understanding of the cultural and healthcare coverage challenges and will follow up on an individual’s holistic needs.
· The CHW will also work on broader outreach, flyer distribution or group education as part of this effort highlighting the need of having individual or familial conversations about vaccinations or harm reduction.
· The CHW will work with the Seniors and Benefits Enrollment team and the CHWs team to provide information, assistance, PPE (KN95 masks, hand sanitizer and other resources) through in-person vaccine and testing events, community events, health and resource fairs, food and resources giveaway events, targeted outreach at local businesses, churches and other entities as well as and on virtual social media platforms.
· The CHW will be responsible for data collection and reporting, including tracking client demographics, number of clients served. Report forms will be provided by NCOA.
· The CHW teams will attend regular COVID-19 meetings or trainings where local statistics are shared and opportunities for networking are encouraged, such as the Chicagoland Vaccine Partnership and potentially alternative methods of getting this information.

ESSENTAIL DUTIES ANDRESPONDSIBILITIES:
- Reaches out to key community stakeholders to provide information about the funded project to reach adults 60 and older and their families and caregivers to improve access to COVID-19 and influenza information, vaccines and boosters and how it impacts their constituents, members, program participants, and / or clients.
- Works with and develops community partnerships to provide educational, informational and COVID-19 and Flu vaccine administration in local venues.
- Assists volunteers with the preparation and dissemination of promotional materials, presentations, documentation, and reporting systems / requirements.
- Develops and maintains formal relationships with local organizations to improve access to programs.
- Other related duties as assigned.
KEY INTERRELATIONSHIPS:
- Senior and Benefits Enrollment staff and volunteers
- Alivio’s Community Health Workers – Health Education and Promotion Department
- Elected officials, government staff, community organizations, healthcare providers, business owners, religious organizations, schools and other entities which have members in need of information and COVID-19 and influenza vaccines and boosters
- Alivio Medical Center program and clinical staff
WORKING CONDITIONS:
OSHA Category 3 – Involves no regular exposure to blood, body fluids or tissues, and tasks that involve exposure to blood, body fluids, or tissues are not a condition of employment
QUALIFICATIONS:
Education:
Bachelor’s degree or combination of education and related experience in outreach or social services programming
Experience:
Preferred three or more years of experience working in the Latino and/or Chinese community
Demonstrated Competencies:
Possesses excellent written and oral communication skills in English and Spanish; or Cantonese and/or Mandarin interpersonal skills; has knowledge, training and/or experience in Windows-based computer environment; familiarity with community- based organizations and institutions that provide or could provide resources for neighborhood residents
