POSITION SUMMARY:
Under the direction of the Pharmacy Operations Manager / Administrative Pharmacy Specialist or designee, performs targeted clinical duties necessary to provide safe and effective pharmaceutical care for both inpatient and outpatient populations. Provides clinical pharmacy services in accordance with required state and federal laws and regulations. Coordinates and administers clinical activities for each respective campus by supervising Clinical Pharmacists (I) and participating on institutional committees. Supervises the activities of pharmacy technicians and interns in assigned work areas. Precepts and mentors advanced pharmacy practice experience students.
ESSENTIAL FUNCTIONS:
- Provides all aspects of pharmaceutical care to patients.
- Performs all job functions of a Clinical Pharmacist I as listed on the most current job description as required.
- Attends daily patient care rounds, where applicable. Acts as a decentralized pharmacist liaison for pediatric, inpatient, and surgical services.
- Quickly and accurately responds to drug information requests posed by the medical and nursing staff.
- Conducts medication reconciliation for both adult and pediatric populations when required
- Demonstrates knowledge and skills necessary to appropriately service the needs of patients
- Responds to and participates in both adult and pediatric STAT call and Code Blue situations
- Formulates compounded preparations not commercially available when appropriate
- Oversees day to day clinical pharmacy operations
- Works closely with the Pharmacy Operations Manager / Administrative Pharmacy Specialist in identifying and resolving pharmacy workflow and performance issues. Supervises Clinical Pharmacists (I), pharmacy technicians, pharmacy interns, and volunteers in the absence of the Director of Pharmacy and Clinical Pharmacy Specialist.
- Provides direction and guidance to Clinical Pharmacists (I) staff in following policies and procedures and fosters the development of their clinical acumen and professional behavior and attitudes.

- Provides in-services and presentations to pharmacy staff as required.
- Serves as a clinical resource for Clinical Pharmacists (I).
- Provides accurate and update drug information to institutional staff
- Acts as a Drug Information Coordinator for pharmacy, nursing, and medical staff. Summarizes and reports new product information as well as providing current information on existing medications.
- Provides in-services and presentations to nursing and medical staff as required
- Develops, implements, and monitors pharmacy driven quality assurance measures
- Works with the Clinical Pharmacy Specialists, Medical Staff, and Nursing to develop and implement quality assurance measures.
- Assesses pharmacy and nursing adherence to established policies and procedures.
- Participates on institutional committees
- Represents pharmacy on the Pharmacy and Therapeutics Committee, Infection Control Committee, Controlled Substance Oversight Committee, Accreditation Taskforce, Hazardous Medication Committee, Environment of Care Committee, Code Committee, Safety Committee and other relevant committees and task forces.

Clinical Performance:
- Responds to drug information and poison information requests and completes appropriate documentation as required.
- Documents all clinical interventions in the Electronic Health Record (EHR).
- Maintains up to date knowledge of sterile and non-sterile compounding as detailed in relevant USP Chapters and state and federal regulations.
- Maintains detailed and updated knowledge, including therapeutic treatment options, for commonly encountered ophthalmic and ENT related diseases states
- Interacts with patients to assess medical and medication history.
- Provides clinically appropriate patient counseling.
- Effectively communicates with medical and nursing staff when addressing patient care needs.
- Coordinates at least one planned presentation for nursing/medical/pharmacy staff per year.
- Directly supervises and precepts advanced pharmacy practice experience (APPE) students

Education
- Attends and participates in departmental staff meetings, weekly huddles, management rounds, and continuing education activities.
- Participates in quality improvement committees, programs, and student presentations.
- Completes a minimum of 20 CE credits annually. Including a minimum of 5 hours related to sterile compounding and 2 hours related to pharmacy law.
- Maintain professional expertise through membership and participation in professional organizations.
- Presents a minimum of 4 in-services to pharmacy, nursing, and/or medical staff annually.
Regulatory Compliance
- Complies with all State and Federal rules, regulations and laws which govern the practice of pharmacy.
- Completes annual MEE regulatory compliance training
- Awareness and application of Joint Commission Medication Management standards
- Maintains Massachusetts pharmacist registration in good standing
- Adheres to all departmental and institutional policies and procedures
Performance Improvement
- Assists the Director of Pharmacy, Pharmacy Operations Manager / Administrative Pharmacy Specialist, and Clinical Pharmacy Specialist in developing and implementing quality assurance measures.
- Develops and presents at least one formulary request proposal, medication administration guideline, or drug use evaluation per year.
Call Responsibilities:
- Required to take weekly call on a rotating basis with other Clinical Pharmacists.
- Call duty will be assigned during times when the Main Campus Pharmacy is closed. Responsibilities include, but are not limited to, the following:
- Answering advanced clinical questions that cannot be answered by contracted order entry pharmacists
- Report to the hospital to compound sterile preparations that may be ordered by authorized providers during off-hours
- Triage temperature excursions that may occur for refrigeration or freezer units that contain medications
- Coordinate the borrowing of urgently needed non-formulary medication
Committees:
Clinical Pharmacists II may be assigned by the Director of Pharmacy to participate on the following committees:
- Pharmacy and Therapeutics Committee
- Hazardous Medication Committee
- Code Committee
- Controlled Substance Oversight Committee
- Antimicrobial Stewardship Committee
- Environment of Care Committee
- Institution Safety Committee
- MGB Enterprise Wide Committees
- Serves as an alternative pharmacy representative on other committees as designated by Director of Pharmacy or Clinical Pharmacy Specialist as required.
EDUCATION AND EXPERIENCE:
- Doctor of Pharmacy (PharmD) plus 3 years of hospital experience or post-graduate residency (preferred)
or
Bachelor of Science (Pharmacy) plus 7 years of hospital experience
- Board Certification in Pharmacotherapy (Preferred)
- Current Massachusetts Pharmacy Registration
WORKING CONDITIONS:
- Works within the Pharmacy Department primarily but will routinely visit patient care and surgical areas
- May be exposed to infectious diseases, carcinogenic, hazardous and sensitizing medications and chemicals
- Works under constant time constraints and deadlines.
PHYSICAL REQUIREMENTS:
- Standing or walking 70% of the time. Lifting and carrying heavy supplies about 5% of job duties.
