Job description – Claremont Home Provider

Independent Services Network (ISN) is a vibrant statewide company providing services for adults with developmental and acquired disabilities, as well as children and youth in foster care.

Summary

ISN is looking for a caring person or couple to support a young active woman who is artistic, funny and full of energy with a great sense of humor. ISN will provide training, oversight and support to the in-home provider and the individual. The in-home provider will reside with this individual in a three-bedroom, well maintained residence located in a quiet country setting, five minutes from downtown Claremont, NH.

This person will model appropriate routines of independent living and interpersonal skills, and provide a warm and safe environment. The ideal home provider will work with the individual to process their emotions and provide coaching to reduce interpersonal conflict. In this way, the support person will work with the individual towards accomplishing their set goals and assist in continuing to build positive relationships with the community. The individual may need guidance with in-home tasks like cooking, housekeeping, laundry, and will need support accessing the community for activities. The individual does participate in a day service program, for 30 hours/week, from Monday through Friday. This program also has built in 30 hours of respite residentially to keep up with her busy active schedule.

Requirements

- Experience working with adults with mental health diagnosis.

- Good written and verbal communication skills, especially for reporting and documentation, as well as for keeping the individual’s supporting team informed.

- Be able to pass a medication administration class.

-All applicants must pass criminal, DMV, Bureau of Adults and Elderly Services (BEAS), and national background checks. Must have a valid driver’s license.

Compensation

-A bi-weekly stipend, based upon the individual’s needs. Housing expenses to be paid by provider.

Job Types: Full-time, Contract

Pay: $71,000.00 per year
