Case Manager/Intake Coordinator position.

• The Case Manager is responsible to ensure the collaborative process of assessment, planning facilitation and advocacy for options and services to meet the patient's health needs through communication and available resources to promote quality outcomes.

Requirements:

• Valid Nurses license
• Case Manager/Intake experience is required
• Skilled in the use of Home Health data programs such as Data Soft Logic.
• Excellent verbal and written skills.

Duties and Responsibilities: Coordinating care between patients, doctors, agency field staff, DME and supply companies as necessary for the patients needs

• -Gather SOC/ROC/Recert reports from field staff regarding new admissions
• -Sheduling necessary staffing
• -Determining valid/justifiable frequency's based on patients case/requirements
• -Trouble shooting problems encountered by field staff
• -Notifying administrator/clinical director of any patient grievances/problems/notifiable incidents regarding patient care
• -Monitoring patients episodes to make sure documentation is complete and submitted on a timely manner
• -Coordinating with the agency's QA regarding review of clinical records submitted by nurse/therapists
• -Ordering and monitoring delivery of DME or medical supplies to patients

All case managers need case manager/care coordinator experience and will receive additional training on agency guidelines and operations when hired.

Job Type: Part-time

Pay: $30.00 - $38.00 per hour

Schedule:

• 8 hour shift

• Microsoft Office: 1 year (Preferred)
