Overview

CleanSlate is a national addiction medicine practice that uses medication to treat patients with Alcohol and Opioid Use Disorder. We also treat associated polysubstance use disorder, depression/anxiety, and Hepatitis C. We utilize addiction medicine best practices in the care of our patients, including the ASAM 6 Dimensions of Care and national practice standards.

We are seeking compassionate, experienced individuals to join our team in our fight against addiction! The Care Coordinator provides support to patients by working with them, the clinical team and other recovery support staff to identify needs and facilitate connections to resources, supports and services available in the community. The Care Coordinator maintains open communication between all parties involved in patients treatment to ensure the highest quality of care is obtained with close collaboration and minimal interruptions in care. The Care Coordinator works with patients to identify their own individualized recovery goals, and resource connections as well as support provided are guided by the individuals own goals. The Care Coordinator utilizes tools such as the Needs Assessment, Brief Assessment of Recovery Capital (BARC-10) and Treatment Plan. Ultimately, the Care Coordinator role is to ensure patients are receiving holistic support and that all parties involved in patient care are cohesively collaborating.

Performance Responsibilities

  • Collaborates with patients, providers, Certified Recovery Specialists, office support staff, ancillary departments within the company, and community agencies to maintain accurate and timely data exchanges between all entities involved in the patients care.
  • Provide care coordination services to patients; making referrals to appropriate community resources to assist patients in dealing with the barriers they face in areas such as housing stability, legal challenges, access to transportation, physical health needs, economic stability, community integration and access to social support, leisure and recreation, etc.
  • Meets with each new CleanSlate patient to welcome and orient them to the program as well as identify needs, strengths, recovery goals and barriers.
  • Facilitates groups on topics relevant to patients served.
  • Obtains appropriate releases for each referral that is completed and conducts follow-up for referrals made.
  • Coordinates patient appointments, referrals, and transportation needs for both CleanSlate and community appointments as needed.
  • Facilitates placement for alternative or higher level of care as agreed upon with the Interdisciplinary Team; maintaining contact with these placements sites to provide continuity of care upon discharge.
  • Maintains effective, pro-active, and superior follow-up communication with patients, providers, office staff, and ancillary departments within the company and community.
  • Enters all required data into the EMR system in a timely and accurate manner.
  • Maintains excellent level of documentation and note keeping.
  • Maintains compliance with all levels and forms of patient confidentiality.
  • Attends regular meetings or conference calls, including but not limited to involving Interdisciplinary Teams and collaborating community agencies; completing all required documentation in the EMR.
  • Provide crisis intervention, as necessary.
  • Maintain individual schedule of assigned patients.
  • Other duties as assigned.

Qualifications

  • Bachelors Degree in a Human Services field required. In PA, Bachelors Degree is preferred.
  • Excellent written and verbal communication, organizational, and customer service skills.
  • Knowledge of community resources, networking, and referral processes.
  • Experience in a Case Management, or Care Coordination setting.
  • Knowledge of and proven experience navigating and documenting in an EMR system.
  • Ability to work independently with minimal supervision.
  • Ability to handle multiple tasks simultaneously and prioritize based on severity of need.
  • This position must successfully pass a pre-employment drug screening and background check

Physical and Environmental Demands:

  • Dependent body mobility to access a standard office environment with shelves and drawers of varying heights.
  • Vision sufficient to read a computer screen, and printed text and handwritten documents.
  • Manual dexterity to operate a computer keyboard on a daily basis.
  • Stamina to sit and/or stand up for hours at a time.
  • Ability to transport, lift or carry essential work-related materials or items up to 40 pounds.
  • Ability to speak and hear sufficiently to communicate clearly in person and over the telephone, and in various forms of written communication.
  • Mental stamina for problem solving.
  • Ability to prioritize multiple tasks.
  • Ability to document, recall, and explain detailed trainings, conversation, or events, and apply oral/written instructions, as needed.
  • Will be required to work in an office environment and be able to travel to other site locations and applicable locations in the community.
  • Must have a reliable vehicle and clean driving record.
