Behavioral Health Case Manager


Location: 1233 Edgewater Street NW, Salem OR 97304

Job Status: Full-time, Monday – Friday, 8am – 5pm

OUR ROLE:

Through positive support, motivational techniques, and encouragement, you will provide strength-based case management to clients enrolled in care at our West Salem Mental Health Clinic. You will work directly with providers and clinicians to provide support services and coordination of care to severe and persistent mentally ill clients.


A trauma informed and collaborative approach will be keys to success in this role. You will have opportunities to make a significant, positive impact as you work alongside agency team members, and community partners in our shared goal of stabilizing and improving the health and social outcomes of our community.


SPECIFIC DUTIES:

  • Assists clients with the development of a natural support system including mentors, coaches, peer support and family members. Establishes rapport, systematically gathers data, determines the client’s readiness for treatment and change.
  • Assists clients in development and implementation of a care plan tailored to the client's identified needs and concerns, designed to improve basic functioning in daily and community living. Identifies for and refers to resources specific to those needs and concerns.
  • Acts as a liaison, client advocate and coordinator among program staff who work with the client's care including any Northwest Human Services program staff and/or other essential supports including medical, community placements, corrections, and mental health providers.
  • Assists clients in attendance and compliance with medical/mental health appointments.

QUALIFICATIONS:

  • Bachelor's degree in a behavioral sciences field; or a combination of three years' relevant work, education or experience for QMHA eligibility.
  • Familiarity with mental health environment.
  • Knowledge of community resources and support systems.
  • Ability to work independently as well as collaborate with a multi-disciplinary team.
  • Bilingual English/Spanish preferred.
