  • Schedule: 4x10-hour DAY Shift variable days and every other weekend
  • Physical Setting: In-office environment. Driving may be required.
  • Work Location: Main office location is located in Lancaster, Ohio. Service area coverage throughout Fairfield, Hocking, Perry, and surrounding counties.

Fairhope Hospice & Palliative Care, Home of The Pickering House is seeking an Admissions Navigator to work with patients, families, and other professionals to promote the philosophy of palliative care and the hospice concept. We believe in a professional team approach.
This position works in collaboration with the Director of Clinical Services and Admissions Coordinator to organize admission information such as history and physical data, referrals, patient visits, physician orders, and other responsibilities.

Responsibilities:

  • Receive, document, and facilitate all inquiry and referrals for patient services in collaboration with Admissions Coordinator.
  • Direct families, patients, and community members to appropriate healthcare providers and available resources.
  • Communicate with health care providers, home health agencies, and physicians to provide information on services available and obtain information for patient referrals.
  • Coordinate admission paperwork and gather patient demographics, history, and physical data, and any additional pertinent information to initiate care.
  • Schedule patient admission with Admissions Nurse, Social Worker, or on-call clinician.
  • Accurately document and communicate all physician orders with nurse case managers and supervisors as appropriate.
  • Coordinate all referrals, admissions, and required patient visits with clinical staff as needed.
  • ·Document electronically any direct services provided and ongoing communication with patients, families, and community providers.
  • ·Remain HIPAA compliant at all times.

Qualifications:

  • A current LPN or CMA license for the state of Ohio is required.
  • Computer skills are required.
  • This position requires proficiency in clinical support functions, such as clinical documentation, faxing, copying, etc.
  • Experience working with end-of-life care and/or healthcare setting preferred.
  • The qualified candidate must demonstrate strong communication and social interaction skills, as well as, be a compassionate individual to work with other professionals and patients and families who face a vulnerable time in their lives.

Job Type: Full-time

Pay: $23.00 - $29.00 per hour
