Position Overview:
The Admissions Referral Coordinator serves as the initial point of contact for individuals in the community seeking services and support. They are responsible for effectively communicating with individuals interested in services and other referral sources, entering screening and referral information into the agency's electronic medical records system, and ensuring clear communication with the field regarding new referrals for timely processing.
DUTIES AND REPONSIBILITIES:
- Serve as the primary point of contact for all individuals in the community seeking services and support.
- Communicate effectively with individuals seeking services and other referral sources to determine their needs and provide appropriate information.
- Enter screening and referral information into the agency's electronic medical records system in a timely and accurate manner.
- Ensure clear communication with the field regarding new referrals for timely processing and follow-up.
- Provide timely assistance or connect individuals with a provider within the required time frame for emergency referrals.
- Track referrals from time of receiving the referral to a client being admitted into a service line
- Track trends and data for referrals, CCA appointment show rates, and other data related to referrals and referral sources.
- Educate individuals about the agency's services and other community resources to help them achieve their goals.
- High School Diploma or higher
- Possess a minimum of 3 years’ experience working with individuals meeting the program admission criteria.
- Possess a valid, regular (non-probationary) state driver’s license.
