POSITION TITLE: ACT Case Manager, 1
CERTIFICATION: QMHA
REPORTS TO: PSR and ACT Team Lead
FLSA STATUS: 1 FTE Exempt

POSITION SUMMARY: Provide skills training and case management services, including but not limited to: providing support to maintain the consumer’s preferred level of independence and community integration; psychosocial skills training; and supportive counseling to individuals with severe and/or persistent mental illness and their families.
Distinguishing Characteristics: Be patient and comfortable relating to individuals with severe or persistent mental illness. Be proficient in coordinating services with care providers from other agencies. Willing and able to provide adequate written documentation of services rendered in order to satisfy Federal and State requirements.
ESSENTIAL DUTIES:

Encounterable Services (50% approx. 20 hrs/week):
Work closely with clients, client’s families and community partners to obtain services for clients including financial and housing assistance, transportation, mental health services, medical services, social services and educational services in accordance with goals set forth in individualized care plan.
Deliverable: Schedule approx. 8 hours of case management per week – making contact with approx. 16 consumers

Facilitate and/or conduct training sessions (individually or in a group setting) to assist consumers in maintaining or achieving desired degree of independence. Sessions will focus on daily living activities to help consumer develop and/or maintain self-management skills such as personal hygiene, grooming, personal spending, and maintenance of personal space and/or living environment, community access and self-expression in accordance with goals set forth in individualized care plan.
Deliverable: Schedule approx. 8 hours of individual skills training per week – making contact with approx. 16 consumers. Facilitate two 1-hour groups per week – making contact with approx. 10 consumers

Assist consumers with medication management and monitor medical issues, following physician’s orders for all medications and/or procedures, and providing documentation of these services (Club Med and COD)
Deliverable: Facilitate Club Med participation for consumers = approx. 4 hours per week as needed.

Paperwork (20% approx. 8 hours/week):
Provide times and accurate documentation for billable services provided to clients in accordance with WVCW, State and Federal policies
Deliverable: All billable services are entered into OWITS each week

Communicate updates using technology platforms including, but not limited to GoogleDrive, FirstClass, OCHIN
Deliverable: All platforms are updated daily

Miscellaneous (30% approx. 12 hours/week):
Team Meetings
Deliverable: Participation in daily team meetings resulting in documented status updates and plans for follow up

Supervision
Deliverable: File indicates that monthly and weekly supervision requirements are met

Become familiar with mental health policy and practice (including 40 hours of training/year)
Deliverable: Participation in 32 hours of off-site training (or RELIAS) per year (one day per quarter). Participation in at least 80% of onsite trainings provided
QUALIFICATIONS
-
Some experience with and/or understanding of the special needs of persons who have a history of mental or emotional problems.

Good writing and speaking skills.
-
Past experience working in the human services field.
-
General familiarity with mental health terminology.

Other desirable personal characteristics would include tolerance, tact, the ability to control emotions in stressful situations and demonstrated functional maturity.
EXPERIENCE AND TRAINING
-
A bachelors degree in a behavioral sciences field, or a combination of at least 3 years relevant work, education, training or experience and has the competencies necessary to communicate effectively;

Understand mental health assessments, treatment and service terminology and to apply the concepts;

Provide psychosocial Skills Development;

To implement interventions prescribed on a Individual Services and Support Plan within their scope of practice;

Ability to perform the duties listed under "Principal Duties and Responsibilities".
SUPERVISION RECEIVED

Person will be supervised by the Assertive Community Treatment Team Lead.
WORK ENVIRONMENT: Work is often performed in settings outside of the normal office environment to include other community offices, client's homes, jail, and the hospital. This position may be exposed to the everyday risks or discomforts which require normal safety precautions typical of such places as an office (i.e., moving mechanical parts, airborne particles, and electrical shock). This position may also be exposed to individuals and situations that are potentially dangerous. Employee will receive training on how to defuse situations and will receive information about agency policy around safety precautions and appropriate response to crisis situations.
OTHER REQUIREMENTS: individual must have a valid driver's license and be insurable. Individual should be able to work flexible hours to include being called at night or on the weekend to provide support to individuals in crisis. Average number of hours a week worked will be 12 hours, with the possibility of an additional 4 hours of administrative responsibilities.
CONFIDENTIALITY: This position requires the handling of highly confidential information. Must adhere to rules and laws pertaining to client confidentiality.
COMMUNICATION SKILLS: Must be able to communicate information in both oral and written form with the ability to present and exchange information internally across teams and co-workers, and externally with clients and the public, Information communicated ranges from routine/ basic information to complex and confidential information
LEADERSHIP & TEAMWORK: Must be able to work effectively and constructively with other office staff, clinicians, the administrative team and all other staff. Participates in staff meetings.
