Occupational Therapy Home Health Job description:

Rehab Accomplished is a therapist-owned staffing agency serving Lewisville TXand surrounding areas. We provide physical therapy, occupational therapy, and speech therapy services. Our mission is to provide quality and evidence-based intervention and make difference for others.

We will love to hire an experienced OT and new Grads, training, and strong support will be provided as needed. At some point, we all are new grads and I believe only therapist can understand and cater to another therapist's work needs so contact us and speak directly with an experienced therapist for further questions.

ESSENTIAL DUTIES AND RESPONSIBILITIES
The following duties are normal for this position. This list is not to be construed as exclusive or all-inclusive.

• Upon receiving OT orders, review member, resident, or patient’s history and completes a thorough assessment, including precautions.
• Communicate assessment/ screening results to members or patients and appropriate family and/or staff.
• Complete required assessment/screening documentation.
• Identify and prioritize needs based on assessment and establish an appropriate plan with goals.
• Collaborate with health care team members to begin planning for appropriate and timely discharge or transfer.
• Demonstrate ability to prioritize treatment procedures according to patient’s needs and MD requests.
• Implement interventions according to the plan of care.
• Communicate treatment plan/appropriate interventions with assistants, initially and ongoing.
• Communicate ongoing treatment issues/plan to interdisciplinary health care team members.
• Complete required documentation regarding daily intervention.
• Document appropriate reassessment and changes in the plan of care.
• Communicate significant changes in intervention patient /family and health care team members.

Qualifications/Skills:

• Master’s degree in Occupational Therapy from an accredited college or university, or graduated from an accredited Occupational Therapy program before 2007 with a Bachelor’s Degree.
• Occupational Therapist license in good standing by Texas State Board of Occupational Therapy Examiners
• Valid Licensed Occupation Therapist by TX Board
• Current CPR certification
