Responsibilities: Job Summary
The Community Services Specialist will be responsible for increasing access to community programs and services through application and enrollment assistance and connectivity to additional resource/supports through community partnerships. The Specialist will be responsible for maintaining expertise in eligibility and enrollment rules and procedures and have a basic understanding of the needs of underserved and vulnerable populations. The Specialist will coordinate and assist with Medicaid/AHCCCS applications and other appropriate public health programs and be responsible for providing client assistance with other personal determinants of health and wellbeing.

• Performs a variety of responsibilities including but not limited to managing community resources to support the needs of clients. Knowledge is maintained of principles and practices of eligibility determination for the following programs: Medicaid (AHCCCS), Nutrition Assistance (SNAP), and Temporary Assistance for Needy Families (TANF), and other public and non-profit support programs.
  
• Provides optimum customer service through consultation, resource navigation and financial counseling/coaching.
  
• Collaborate and coordinate outreach efforts with community partners, and provides “closed loop referral” data reporting to partners for evaluation of program impact.
  
• Responsible for completing the mandatory Health Insurance Market Place Training and maintaining current client files and operating reports.
  
• Supports the Desert Mission Food Bank with front intake/client relations and community resource navigation as needed.
